英文摘要 | Objective: Heroin addicts have a high rate of relapse during rehabilitation, and episodes of relapse are often related to depressed mood. This exploratory investigation sought to assess the self-attributed factors for depressed mood in heroin abusers, and to thereby develop a strategy for coping with the high risk of relapse. Methods: All cases admitted for heroin abstinence in our psychiatric hospital from May 1997 to July 1997 were considered eligible for the study. An exploratory in-depth interview was conducted by a research psychiatrist to evaluate the emotional status of the subjects before and after the heroin abuse. Mood disorders were diagnosed according to the criteria in the DSM-IV and ICD-10 classification. Self-attributed factors related to depressed mood including social, occupational, family and peer relationships, and value over drug use were analyzed to determine their relationship to the development of depressed mood. Results: A total of 15 subjects completed the interview. All subjects were found to have varying degrees of depressed mood after using heroin and 3 of them met the diagnostic criteria for major depressive disorder in the DSM-IV or moderate and severe depressive episodes in the ICD-10. Self-attributed factors accounting for the depressed mood included guilty feelings over heroin use, deterioration of occupational and social function, family conflict, legal problems, and feelings of being unaccepted or being looked down upon by society. Conclusion: Depressed mood was common among heroin addicts. A systematic inquiry into the causal relationship between depressed mood and relapse is important for the development of successful rehabilitation programs for heroin addiction.
