中文摘要 | 關於棉塵的危害性就目前所知主要為病理所見的慢性支氣管炎,甚至肺氣腫,肺 功能可見阻塞型肺病、FEV1 逐漸下降,而在一些流行病學研究上, 並沒有結論與肺癌有關 ; Schilling 等人觀察棉紡織工人心血管疾病,腎臟及呼吸道疾病死亡率較高,另外在英 國十年統計的聯業死亡可見胃癌、口腔癌有較高的死亡率, 而 Henderson 等人觀察紡織工 作鼻腺癌也有較高的死亡率。自 1930 年來許多學者開始著手棉紡織工廠死亡率的世代追蹤 ,以了解棉紡織廠族群各種疾病死亡率的情形,但大部份的研究結果顯示各種疾病的標準死 亡比 SMR (Stamdard Mortality Ratio) 多半小於 1,本文的目的是運用集體資料分析的方 法來作一綜合性的文獻回顧。 結果如下:總死亡 Meta SMR 男性 =0.792,女性 =0.873 都 低於 1, 關於所有癌症 Meta SMR, 男性 =0.705 較低, 女 =0.829; 關於循環系統疾病 Meta SMR,男性 =0.849,女性 =0.937,如果考量 " 健康工人效應 " 此因素,循環系統死 亡之 Meta SMR 大於 0.8 便有意義,尤其是女性; 關於呼吸癌症 Meta SMR,男性 =0.599 非常低關於呼吸道疾病 Meta SMR,男性 =0.67 較低,如果考量 " 健康工人效應 " 因素, Meta SMR 小於 0.75,表示棉紡織廠工人比一般人健康之外,可能尚有其他因素:一. 研究 族群排除了較嚴重的呼吸道疾病而剩下非致命性呼吸道疾病的人來納入研究。二. 棉塵可能 產生人體免疫反應來抗癌。 |
英文摘要 | Pathologic change in cotton textile workers may be chronic bronchitis and emphysema. In lung function test, it may perform obstructive lung disease and FEV1 decreased. In epidemiologic studies, no relationship was found between cotton dust and lung cancer. Schilling et al. observed cotton textile workers had high mortality rate about cardiovascular disease, renal disease, and respiratory disease. In British ten years statistics of occupational deaths, high mortality rates about gastric cancer and oral cancer were found in cotton textile. Henderson et al. observed high mortality rate of nasal adenocarcinoma in cotton textile. From 1930, many cohort studies began to find the disease mortality rate in cotton textile population. But almost all results of studies were low SMR (standard mortality ratio). We use meta-analysis to review disease and cancer mortality rates in cotton textile workers. The results as below: All deaths meta SMR in male =0.792, in female =0.873 About cancers meta SMR in male =0.705, in female=0.829 About cardiovascular diseases meta SMR in male =0.849, in females =0.937 If we consider "healthy worker effect", it is significant higher than 0.8 in female cardiovascular disease meta SMR. About respiratory cancers meta SMR in male =0.599 About respiratory disease meta SMR in male=0.67 The causes not only healthy worker effect but also other reasons may effect the results. First, Some studies rule out severe respiratory disease and contain only nonfatal respiratory disease in the study population. Second, it is possible that the agent that produces symptoms of byssinosis or something else in cotton dust could stimulate an immunological response against cancer. |
