英文摘要 | Background. Cardiac arrhythmias and respiratory arrest are the most frequent causes of immediate death from electrical injuries. Ventricular fibrillation (VF) or ventricular asystole may occur as a result of electric shock. Respiratory arrest may occur secondary to electric current that injures the medullary respiratory center or paralyzes respratory muscle or the diaphragm. We report on a patient who presented with dead on arrival (DOA) induced by electric shock, who survived after vigorous resuscitation. Methods and Results. A 24-year-old previously well man who was transferred to our emergency department due to cardiopulmonary arrest caused by an accidental low-voltage (110v) household alternating current shock at work. On arrival at the emergency room, he presented with DOA with electrocardiogram (ECG) monitor showing VF. Over the next 30 minutes, the patient received six external defibrillations and endotracheal intubation until stable sinus rhythm was achieved. Although the patient experienced nosocomial pneumonia complication during hospitalization, he regained consciousness and recovered well after tracheotomy and 6 weeks of antibiotics treatment. Conclusions. Based on our observation, victims with electric shock can be resuscitated successfully by aggressive and sustained advanced cardiac life support (ACLS) procedures without delay, even those presenting with DOA. |
