中文摘要 | 背景:連續性腎臟替代治療已成為危急病人合併急性腎衰竭之重要選擇之一,本 報告乃對於40位多重器官衰竭危急患者合併急性腎衰竭接受連續性靜靜脈血液透析治療之 研究與分析。 方法:40位患者皆於加護病房照顧,患者平均年齡50歲(21-82歲),平均器官衰竭數 目為4.9個(4-6)全部患者皆需呼吸器支持,連續性靜靜脈血液透析乃經由雙腔透析管插入股 靜脈來進行,其血流速度由幫浦來帶動整系統並維持在每分鐘120-150公撮,含碳酸鹽之透 析液以每小時700-900公撮速度經由點滴幫浦輸送至過濾器,並記錄其治療當中之併發症。 結果:結果顯示平均尿素氮清除率為每分鐘14.7公撮,平均肌酸酐清除率為每分鐘15.4 公撮,電解質包括鈉、鉀、氯、鈣、磷皆在正常範圍。兩位患者發生體外迴路血液凝固,1 位患者發生體溫過低現象。計有7位病人存活,其存活率為17.5%。 結論:連續性靜靜脈血液透析治療,對於多重器官衰竭危急患者合併急性腎衰竭為一安 全、有效、容易設立之治療。其提供危急病人合併急性腎衰竭良好的代謝、電解質及體液之 控制,並給予危急病人合併急性腎衰竭提供良好的環境以對抗其潛在疾病。 |
英文摘要 | Background: Continuous renal replacemnet therapies have become the preferable methods for treating critically ill patients with acute renal failure. We studied the use of continuous venovenous hemodialysis (CVVHD) for the management of multiple organ failure patients with acute renal failure. Materials and Methods: Forty patients in intensive care units were studied. Their mean age was 50.5 (range 21 to 82) years. The mean of number of organ system failures per patient was 4.9 (range 4 to 6). All patients required mechanical ventilation. CVVHD was performed from the femoral vein via a double-lumen catheter. Blood flow was maintained via blood pump at rate of 120 to 150 ml/min. Bicarbonate solution was delivered to the dialysate ports of the filter at rate of 700 to 900 ml/hour. Any complications during therapy were recorded. Results: The mean urea clearance was 14.7 ml/min and the mean creatinine clearance was 15.4 ml/min. Electrolytes were all within the normal ranges. These were two episodes of extracorporeal circuit blood clotting and one patient experienced hypothermia. Seven of the 40 patients (17.5%) survived. Conclusion: CVVHD is a safe, effective, easily established therapy for managing the multiple organ failure patients with acute renal failure, offering good metabolic, electrolyte, and fluid control. |
