中文摘要 | 腦血管病變往往造成患者感覺運動障礙,其中半側偏癱是腦血管病變最常見的症 狀。肢體功能的重建對半側偏癱患者極其重要,但半側偏癱患者常由於過於依賴健側,造成 站立時體重的轉移不良,甚至有全靠健側載重的情形,此種現象對於步行重建之病患有顯著 影響,會造成日後不良的步態。本研究利用視聽迴饋平衡訓練器,提供視覺和聽覺的生物迴 饋刺激,加強大腦再整合,以期達到改善半側偏癱患者功能性動作。研究以十位離腦血管病 變時間六個月以上之側偏癱患者,分控制組及實驗組進行,控制組接受傳統患側載重和重量 轉移的物理治療,實驗組除了傳統物理治療外,尚須接受視聽覺的迴饋平衡訓練;兩組於一 個月治療的前後, 以步態分析和測力板量測其站立平衡和步態資料以為評估。 本研究利用 Mann-Whitney U test 統計分析發現: 實驗組在站立穩定度比控制組有明顯進步( P<0.05 )。 |
英文摘要 | Stroke patients with hemiparesi are usually suffered from blance impairments. An uneven weight bearing distribution, which leads to asymmetry characteristics of the locmotor pattern, is commonly observed among these patients during quasi-static stance. According to the biomechanical and neurological control theory, improving the balance control and interlimb weight transfer capability could contribute to a more symmetrical and effective walking pattern. The purpose of this research was to investigate the effects of using a visual and auditory biofeedback system in blance retraining program on stroke patients. Ten hemiplegic patients with at least six months of onset are participated in this study. The patients were randomly divided into two groups with five patients in each grouptratment group recieved biofeedback blace fraining and whil control group the conventional physical therapy procedureonly received the conventional physical therapy procedure only. Using a forceplate and motion analysis system, standing blance and locomotor performance parameters were recorded for each subject during two evaluation sessions before and after the one month treatment period. The data were analyzed by Mann-Whitney U test to assess the effects of biofeedback balance training. The results showed that subjects with the biofeedback training have significant improvement in static standing stability than those subjects without the feedback training (p<0.05). Compared to the data prior to training, both groups have showed significant improvements in static standing symmetry, overall locomotor performance, gait pattern, and interlimb coordination after training. However, the augmented biofeedback training provided insignificant effect (P>0.05). These might be due to the small sample size with subject variations and limited training period. |
