中文摘要 | 本研究目的在於:1.瞭解居家臥床病人特性、主要照顧者特性、壓力程度、因應行 為及身心健康的現況,2.探討主要照顧者的壓力源,3.分析主要照顧者的壓力源、因應行為 與身心健康之關係及預測力。研究對象是以北部某準醫學中心居家照護所有收案個案之主要 照顧者,共計64位。經結構式問卷收集資料,並用因素分析、描述性統計、皮爾森積差相關 及逐步多元迴歸來進行資料分析。本研究結果發現:「生活作息的限制」、「照顧病人的知識不 足」、「社會支持的缺乏」、「家庭面臨的衝擊」以及「照顧責任的負荷」,是主要照顧者面臨的 五大壓力源,其中又以「生活作息的限制」最常發生。在使用因應行為上,以「被動接受型」 最多。而主要照顧者的身心健康情形尚可,但以出現「憂鬱反應」的症狀較多。在壓力源與 因應行為、身心健康的相關性中,當主要照顧者感受到壓力事件的頻率愈高時,使用因應行 為的頻率愈高,且身心健康的狀況愈差。而且使用負向或消極的因應行為愈多者,其身心健康 的狀況愈差。此外,當主要照顧者感受「照顧責任的負荷」愈大,或經常使用「逃避退縮型」 的因應行為,則可預測主要照顧的身心健康情形愈差。研究者並根據研究結果,提出在居家 護理實務方面的建議。 |
英文摘要 | The purposes of this research are 1.to understand the characteristics of bedridden patients at home, and their primary caregivers' characteristics, stress level, coping behavior, and physical-mental health status. 2.to investigate the stressor of primary caregivers. 3.to shed light on the relationship between stressors, coping behaviors, and physical-mental health status, and the prediction value of stressors and coping behaviors. 64 primary caregivers who cared bedridden patients receiving home care provided by Mackay Memorial Hospital were selected for this study and using structured questionnaires to collect data. Data were then analysed by factor analysis, descriptive statistics, Pearson's correlation, and stepwise multiple regression. The results revealed that: the major stressors of primary caregivers are : "limitation of life", "inadequate knowledge of patient care", "lack of social support", "conflict with family", and "burden of responsibility in patient care". The limitation of life is the greatest stress. The "passive acceptance" is the most common coping behavior of primary caregivers. The caregivers physical-mental health status are relatively normal, but they more or less show a kind of anxiety. Stressors are significantly related to coping behaviors and physical-mental health status. The more stressful events the more they will tend to have passive coping behaviors and worse physical-mental health status. Primary caregivers using negative or passive coping strategies more frequently tend to have worse physical - mental health status. "The burden of responsibility in patient care" and the "escapable withdrawal" coping behavior are the significant predictors of caregivers' physical-mental health status. Based on these results, we proposed some recommendations for nursing practice. |
