英文摘要 | A 56-year-old-woman presented with a metastatic mass and lymph-adenopathy involving left buccal region and carotid artery. Her medical history included radiotherapy for nasopharyngeal carcinoma and surgical resection of a metastatic tumor involving the left parotid gland. The patient received neck surgery to excise the tumor mass and lymph nodes enclosing the left carotid artery. Speech disorder followed by right hemipariesis was noted when the patient awoke from anesthesia, indicating early postoperative stroke. Computed tomography findings were normal. However, Tc-99m hexa-methylpropyleneamine (Tc-99m HMPAO) brain single photon emission computed tomography (SPECT) showed diminished cerebral perfusion over the left temporo-frontal lobes and crossed cerebellar diaschisis, concordant with the clinical manifestations of a left supratentorial stroke. Follow-up Tc-99m HMPAO brain SPECT 5 months later revealed progressively decreased perfusion over the left hemisphere. The patient died about 1 year after this perioperative stroke. Our findings in this case suggest that Tc-99m HMPAO brain SPECT may be a useful imaging modality for evaluating perioperative strokes. |
