中文摘要 | 慢性C型肝炎臨床上多以干擾素治療,但治療時易引起副作用,停藥後復發率高 且價格昴貴。 目前也偏向研究合併使用其他藥物如 riavirin,胸腺素等,期望達到較好療 效, 而中藥小柴胡湯經研究證明可促進細胞成熟,刺激 T 胞功能,對多種肝損傷動物模型 呈明顯保護作用,而茵陳、鬱金及車前子,皆能促進膽汁分泌。糾正蛋白倒置,可達到營養 保肝之目的( 18 )而合併入使用。因此本研究以小柴胡湯為主加茵陳、鬱金及車前子為治 療方劑評估療效,並比較副作用及價格,以了解取代干擾素之可能性。本研究之對象為中國 醫藥學院附設醫院肝膽科門診病人中符合條件者。隨機分為加味小柴胡湯組。干擾素組及安 慰組,每組 20 人。治療三個月及六個月後以變異數分析法、Tukey's 事後檢定及羅吉斯方 程式進行評估。 結果顥示加味小柴胡湯能改善慢性 C 型肝炎之臨床症狀,未出現注射干擾 素之副作用,價格亦較便宜且能降低 ALT 及 AST 值,但在 ALT 及 AST 值降至正常範圍之 比率及清除 C 型肝炎病毒 RNA 之效果上不如干擾素。統計資料亦顯示加味小柴胡湯之治療 為緩慢而持續之結果,因此適當加減方劑是值得發展之途徑。 |
英文摘要 | Interferon therapy os the most effective treatment for chronic hepat it is C at present. However, it is expensive and accompanied with side-effects and an un-satis-factory outcome. The present study assessed the clinical effectiveness of jia-wei-xiao-chai-hu-tang on chronic hepatitis C and compared its side effects and costs to examine the possibility of using it as a substitute for interferon. Subjects were selected from the most qualified volunteered patients in China medical College Hospital. The clinical effectiveness of three different groups(2) patients for each group):(1) Chinese Herbs "jai-wei-xiao-chai-hu-tang"; (2) a-interferon; and(3) placebo, were evaluated. All subjects recieived treatment for 6 months and data collected at at 3 and 6 months of the experiment were analyzed by using ANOVA. Tukey's test, and Logistic regression. The results showed that "jai-wei-xiao-chai-hu-tang" was capable to improve clinical symptoms of chronic hepatitis C. without side effects of interferon therapy. Futrhermore, it was also capable to lower levels of ALT and AST,and was cheap. However. the result indicated that interferon wsa the most effective agent among three test groups in lowering ALT and AST levels to normal range, and in clearing-up HCV RNA. In order to develop new Chinese herbs for the treatment of chronic Hepatitis C, further research is required. |
