中文摘要 | 由於牙髓與牙周疾病在組織病理及解剖上均有密切關連,兩者間有許多共同的臨床 徵候,若能準確診斷因,並予妥善治療,即可舒緩症狀,將病患治癒。所以本研究搜集臨床 和 X 光片上,合併牙髓與牙周疾病的牙齒 100 例,依 Weine 分類法,分成四類型。 結果 發現,第一類型 -- 臨床及放射片類似牙周病,其實為牙髓病者,有 47 例。 第二類型 -- 同時具牙髓及牙周病者有 30 例,其中 6 例為牙周感染導致牙髓壞死的病例; 其餘為牙髓 病因造成牙周病者。第三類型 -- 無牙髓病但須先予牙髓治療再予牙根截除者,如�G復不良 、骨質裂縫 (dehiscence) 等,共 10 例。 第四類型一臨床及 X 光片似為牙髓疾病,其實 為牙周病者有 13 例。本研究發現,病因起源於牙髓病者,共有 71 例,占全部病例之 71% 。 經術後一年追蹤顯示,所有病例均無任何臨床不適出現,除 2 例原本牙周狀態較差外, 其他尤其經齒內治療的牙齒大致均有良好癒後。 |
英文摘要 | Because of the interrelationship between histopathology and anatomy, pulpal and periodontal diseases have some common clinical symptoms and signs. A thorough understanding of etiology is necessary for accurate diagnostic, and the delvelopment of appropriate techniques and effective treatment. In this study, 100 teeth with both pulpal and periodontal lesions were divided into four catogories according to the classification of Weine. Forty seven teeth were classified as class I. Although these teeth had symptoms that clinically and radiographically simulated periodental disease, only pulpal disease was present. Thirty teeth were categorized as classⅡ. These teeth had concomitant pulpal and periodontal disease. Only six of these teeth had periodontal infection which had gradually resulted in pulpal necrosis; disease in the other teeth was suspected to stem from pulpal lesions. Ten teeth were categoried as class Ⅲ. These teeth did not have pulpal disease but did require endodontic therapy plus root amputation or planing to allow periodontal healing. Causes of disease in category III teeth included improper prosthesis, dehiscence, and others. Thirteen teeth were categorized as class IV. The clinical and radiographic findings in these teeth, although similar to those of pulpal disease actually indicated the presence of periodontal disease. Of the 100 teeth in this study, 71 had diseases of pulpal origin (71% of total cases). Follow up evaluation at one year revealed no recurrence of clinical symptoms in any of the patients, except in two cases of congenital abnormalities with poor periodontal status. |
