中文摘要 | 本研究共收集某教學醫院急診部 1994 年 1 月至 1995 年 10 月間狗咬傷就診個 案 200 例,針對不同咬傷情況評估其傷口發生感染的相關性。結果發現,有 8 %個案發生 傷口感染,傷口發生感染與否與傷口暴露時間、個案是否同時具有糖尿病具有顯著相關。傷 口暴露時間超過 24 小時者發生感染的比率是 8 小時以下者的 12 倍, 同時具有糖尿病的 個案發生傷口感染的比率為非糖尿病患的 6.4 倍。 在調整年齡、性別、傷口型態及咬傷部 位等因素影響之後發現,傷口暴露時間每增加一個小時,發生感染的機率就增加 6.76 %, 糖尿病個案發生傷口感染的危險性是非糖尿病個案的 2.43 倍。傷口感染與咬傷部位、創傷 型態、年齡及性別均不具顯著相關。由本研究結果得知,除了應提醒民眾預防可能發生狗咬 傷之情況外,在傷口形成後,應儘早送醫處置,尤其是具有免疫力低下之個案,應妥善處理 傷口並給予合適之抗生素治療,以減少發生傷口感染之機會。 |
英文摘要 | We recruited 200 cases of dog-bite from the Department of Emergency Service of a teaching hospital between January 1994 and October 1995. Evaluation of the association between wound infection and various conditions was done. The results showed 8% fo the cases developed infection. There were significant associations between wound infection and exposure time and comorbidity of diabetes. Those who has wound exposure longer than 24 hours had a 12 times risk of infection than those of exposure for less than 8 hours. Those who had diabetes mellitus had a 6.4 times risk of wound infection than nondiabetes. After adjusting for the effects of age, sex, type of wound and location of wound, the risk of infection had a 6.76% increment as the exposure time had an increase of an hour. The diabetic had a 2.43 times risk of wound infection than did nondiabetes after adjustment for the effects of other variables. There were no significant associations between wound infection and wound location, wound type, patient's age, and sex. In shows that besides reminding people to prevent the conditions with high risk of dogbite, it is mandatory to have an earlier management of the wound, especially for those with an immunodefficiency state such as diabetes mellitus. It is necessary to have adequate wound management and adequate antibiotic treatment for this kind of wound to reduce the risk of getting infection. |
