中文摘要 | 腸病毒感染在不同年齡層,有不同的臨床表現及實驗室檢查結果,然而其潛在性的原因則尚未定論。本研究的目標在於檢測同一種病毒對疾病的臨床表徵及實驗數據之影響。最近研究顯示某些細胞激素參與發炎過程,不同的病毒性腦膜炎有不同的細胞激素反應。 為了解病毒作用的差異性,我們探討十二位經由腦脊髓液培養證明為急性ECHO 30病毒腦膜炎兒童的臨床及實驗室反應。我們發現ECHO 30病毒腦膜炎在三歲以上的兒童均有發燒、頭痛、嘔吐的症狀,所有病患白血球增生且以多核白血球反應為主。細胞激素反應有較大變異且無預測模式。Interferon-γ (IFN-γ),及Interleukin-1β(IL-1β),在病患腦脊髓液中分別升高66%及50%。腦脊髓液中未測出Granulocyte-macrophagecolony-stimulating factor(GM-CSF)。由我們的發現可以推測病毒抗原能調節腸病毒疾病的臨床及細胞變化,而病毒性腦膜炎的細胞激素反應則主要受宿主本身因素的影響。 |
英文摘要 | Variations in age susceptibility and in clinical and laboratory features of enteroviral infections are well recognized, but the underlying causes remain undefined. The objective of this study was to examine the influence of an identical serotype of virus on the clinical and laboratory expression of the disease. Recent studies suggest that certain cytokines may participate in the acute inflammatory processes and those variable cytokine responses to different viruses occur in aseptic meningitis. By eliminating the variations in viral agents, we examined the clinical and laboratory responses in 12 children with acute echovirus 30 meningitis proven by cerebrospinal fluid (CSF) cultures. We found that echovirus 30 meningitis produced consistent clinical symptoms of fever, headache and vomiting in nearly all children over 3 years of age and that leukocytosis with a predominance of polymorphonuclear leukocyte responses occurred in all patients. Cytokine responses were variable and showed no predictable patterns. Interferon-γ(IFN-γ) and interleukin-1β(IL-1β) in the CSF were elevated in 66% and 50% of patients respectively. No granulocyte-macrophage colony-stimulating factor (GM-CSF) in the CSF was demonstrated. Our findings suggest that viral antigens may modulate the clinical and cellular changes of enteroviral disease, whereas cytokine responses to viral meningitis might be mostly mediated by the host factor. |
