中文摘要 | 近幾十年來牙周疾病的治療目的,就是設法使原被破壞的牙周組織再生。然而,大部份以 往使用的牙周治療方式在達成此目標的成就上,極其有限。今天在臨床及組織學的證據,證明引導 組織再生術提供了對達成此目標相當的進展。這種術式原理,是在清除過的牙根表面上選擇性的再 分佈牙周組織中一些具有再形成原有組織潛能的細胞。目前研究得知,要完全重建受損的牙周組織, 是不太容易達成的。因在牙周復原的過程中至少有四種不同的細胞參與,例如牙齦上皮細胞、骨細 胞、牙齦結締組織細胞和牙周韌帶細胞。然而許多局部釋放的因子也會調節這過程。換句話說,再 生組織的性質取決於暴露牙根表面及牙周骨缺損處再分佈細胞的種類。因此正確的再生最主要的因 素、就在於參與細胞的種類。很多證據顯示由牙周韌帶來的細胞有很多相容的生物特性,且是能引 發完全牙周組織再生的主要來源。根據這個推論,我們發展了可用於臨床上的細胞移植新技術,以 控制來達成所有牙周構造最佳的癒合狀態。本文簡短的回顧與討論牙周韌帶細胞的生物生化特性, 及其應用在引導組織再生術和細胞移植法的科學原理。 |
英文摘要 | In the treatment of chronic periodontitis, regeneration of the periodontium lost as a result of the pathologic process has long been an optimal goal for several decades. However, most previous modalities of periodontal therapy had demonstrated limited success in achieving this goal. Today guided tissue regeneration (GTR), based on the principle of selective repopulation of debrided root surface by periodontal tissue derived-cells that bear the potential to reform original tissue, offers significant progress in compliance with that goal with convincing clinical and histologic evidence. Recent studies indicated that complete restoragion of the damaged periodontal tissues is not easy to accomplish. This is because four, at least. different types of cells (such as gingival epithelial cells, bone cells and cells from the gingival connective tissue and periodontal ligament)participate the healing of the damaged periodontal tissues, although many locally released factors may modulate this process. In other words, the cells that repopulate the exposed root surface and periodontal osseous defect will determine the nature of the tissue that will regenerate. Thus, the key element for proper regeneration lies in the cell types involved. Many compelling evidence suggest that cells from the periodontal ligament have many compatible biological features and are prominent reservoir that can initiate a complete regeneration of the periodontium. Based on this postulagion, we developed a new cell grafting technique clinically to control the optimal healing pattern in achieving restion of all periodontal structures. The article will briefly review and discuss the biological and biochemical characteristics of the cells derived from the periodontal ligament and the scientific rational of its application in GTR and cell grafting. |
