中文摘要 | 目的︰急診精神醫療是相當重要的臨床工作,國內過去尚未有精神專科醫院急診之相關研究,本文旨在分析北區一所精神科專科教學醫院急診就醫病患,以期瞭解精神科專科醫院急診病患之臨床特徵。方法︰作者使用「急診病人資料登錄表」收集1994年10月至1995年12月期間前往該院急診就診病患之臨床資料,並加以分析。結果︰本研究收集3054人次病患,平均年齡34.8 ±0.2歲;男性53.6%,女性46.4%;就診原因以干擾破壞行為最多(46.5%);臨床診斷以精神分裂病(39.0%)及情感性精神病(20.6%)居多,次之為物質相關疾患(11.4%)。就診時,符合精神衛生法之嚴重病人76.6%,非嚴重病人23.4%;又因該院置有精神科留觀病床,56.3% 病人曾使用留觀病床;經急診處理後,37.4% 的病患接受進一步住院治療,2.1% 的病患因合併內外科問題需轉介綜合醫院,55.8% 的病患,經由急診處理後,可以轉介門診繼續治療。經由急診處理之後,病患離開急診時,其法律狀態有所改變。結論︰本研究顯示精神科專科醫院急診患者嚴重度較高且物質相關疾病佔有顯著比例;留觀的設置與嚴重病人的判斷對整體急診醫療有助益;評估住院與否,危險行為及精神分裂病的診斷是最重要的。 |
英文摘要 | Objective: This study reviews clinical data concerning emergency services provided in a psychiatric hospital. Methods: It was conducted in an emergency room of a psychiatric teaching hospital. Data was collected from October 1994 to December 1995 on 3054 emergency visits. Results: Of 3054 patients, 53.6% were male, 46.4% were female, and the mean age of the patients was 34.8±0.2(male: 34.1±0.3, female:35.6±0.3). The symptoms most frequently demonstrated by patients wee destructive behaviors toward objects, followed by violent behaviors toward people. The most common diagnosis was schizophrenia (39.0%), followed by mood disorder (20.6%), and substance-related disorder (11.4%). According to Mental Health Law, 76.6% were classified severe patients; 56.3% were in the emergency short-stay unit, and among them, 17.7% were put in a seclusion room. After acute intervention, the psychotic severity improved and only 37.4% of patients required admission, while 2.1% needed referral to a general hospital for treatment of other medical conditions. Conclusion: The study suggests:1) There is a steady increase of emergency visits, indicating an increased need for new psychiatric services including emergency services. 2) A short-stay unit is necessary in cases requiring clinical consideration. 3) The assessment of legal status in terms of Mental Health Law is important in clinical diagnoses. 4) Substance-related disorders are becoming the main issue in psychiatric emergency services, especially alcohol-related disorders. 5) Strengthening of the delivery system for psychiatric emergency patients is mandatory. |
