中文摘要 | 高級心臟救命術課程早在1975年就在美國開始被大力地推廣,為了達到最好的救命效果,本課程常常修正,以求提供最新的訊息與達到最佳的教學效果。1994年,本課程做了一重大的變革,那就是全面採用狀況模擬實際操作,來取代以前課堂講課與分組操作的教學方式。我們也改變我們高級心臟救命術的教學方式,然而因為顧慮本課程在國內推廣只不過五年,學員們甚至對其中很多觀念與技能都還不熟悉,因此保留了部份課題於課程前講授,並稱之為「課前課程」。為了評估本地學員對此「課前課程」的需求性以及對「課前課程」與狀況模擬實際操作的滿意度,我們設計了問卷,並且自課程教法變更後開始評估。問卷調查的結果顯示,學員對「課前課程」的需求性頗強,不論他是醫師或是護理人員。進一步的分析顯示出對於「課前課程」的需求性,不論是醫學中心、區域醫院或是地區醫院的醫護人員都很強烈;而此需求性也並未因他是主治醫師、住院醫師或是實習醫師而有所不同。至於課程滿意度方面,所有的學員對「課前課程」與狀況模擬實際操作都很滿意。雖然後者的滿意度較前者稍高,不過其間的差異並未達到統計學上有意義的差距。絕大部份參加過此課程的學員都覺得課程內容相當實用且有幫助,應該大力且全面地推廣。經本研究分析,本地的學員對於「課前課程」需求孔急,因為他們覺得由其中可以學得相當多「新觀念」。臺灣與美國的狀況未必完全相同,在引進美國課程教學時,適當的評估以及就本地之需求來修正是必須的。 |
英文摘要 | The Advanced Cardiac Life Support (ACLS) provider course is a famous training course for medical personnel. It was propagated in United States beginning in 1975. This course has been modified repeatedly in the hope of improving it. In 1994, very substantial changes were made. Only scenario workshops were used instead of standard lectures and workshops. We changed the method of teaching as recommended, but some lectures were preserved as "pre-lectures". This was done because the ACLS course was taught in Taiwan for only 5 years and many participants were not very familiar with the course contents. A questionnaire was designed to assess the necessity of the "pre-lecture" and satisfaction with the pre-lecture and the scenario workshop. There exists a great necessity for the prelecture, for both physicians and nurses, regardless of their grades of hospital or level of training. Course participants were satisfied with both the pre-lecture and the scenario workshops, and there were no significant differences between groups of participants. Generally speaking, they liked this course and wished it was available earlier. The method of ACLS teaching which only uses scenario workshops may work wonderfully in the United States, but our students need the pre-lecture because they lack the necessary background. Adequate assessment and modification if necessary should be encouraged before teaching ACLS provider courses in Taiwan. |
