中文摘要 | 胎兒從懷孕後期起會大量堆積花生四烯酸(AA)及二十二碳六烯酸(DHA)於腦部,推測早產兒因提早出世,體內累積AA、DHA有限,而AA、DHA為生長及中樞神經、視網膜發育所必需。因此我們以臍帶血紅血球細胞膜脂肪酸組成作為神經組織堆積AA、DHA指標,於中國醫藥學院附設醫院收集25名平均孕齡32週之早產兒與35名足月兒臍血,分析脂肪酸組成。結果發現早產兒出生時紅血球細胞膜AA、DHA%顯著低於足月兒,僅有足月兒一半;追蹤十名早產兒至出院,顯示住院期間AA、DHA狀況更形低落,出院時只達足月兒出生值之20∼30%,我們推測可能與餵食不含≧20C長鏈多元不飽和脂肪酸之配方有關。 |
英文摘要 | The demand of the fetal and neonatal brain for AA (arachidonic acid; 20:4(n-6)) and DHA (docosahexaenoic acid; 22:6(n-3)) shows a substantial increase during the last trimester of pregnancy and the first month of postnatal life. Thus a preterm infant born at less than 34 weeks will be deprived of this critical intrauterine lipid accretion. Since AA and DHA are important fatty acids for the growth and development of CNS and retina, the fatty acid composition of red blood cell membrane was used as an indicator of neural accumulation of AA and DHA. Cord blood of 25 preterm and 35 term infants was collected in the China Medical College Hospital. The results showed there were lower AA and DHA percentages in RBC membrane of preterm infants than term infants. The 10 preterm infants followed, AA% and DHA% in RBC membrane dropped from birth to discharge. Four weeks after birth, these levels were only 20-30% of the level of term infants. It may be attributed to the feeding of formula containing no ≧20C long-chain polyunsaturated fatty acids (LCP). |
