中文摘要 | 放射線治療鼻咽癌,可能會引起上頜竇發炎。所以部份病患在照射後會有流膿鼻 涕的症狀發生,下鼻道鼻竇造口術可提供上頜竇另一個換氣及引流的開口,是治療上頜竇炎 的方法之一。我們對 16 名鼻咽癌病患於放射線治療之前,實施下鼻道鼻竇造口術,並置入 一矽質引流管 (Sino Vent),以研究此法是否能預防或減輕放射線所引起的上頜竇炎。結果 顯示,Sino Vent 的放置對放射線引起之上頜竇炎並無預防效果,反而有不利的影響。另外 ,以內視鏡及病理切片觀察鼻竇黏膜在放射線治療後會發生息肉及炎症性變化,而這種變化 是可逆的。 |
英文摘要 | Maxillary sinusitis is one of the complciations after irradiation to the head and neck. Nasal antrostomy is now widely adopted as a treatment modality for maxillary sinusitis. In this study, we performed nasal antrostomy and placed a Sino Vent on the maxillary sinuses of 16 nasopharyngeal carcinoma patients prior to radiotherapy to investigate if this procedure could prevent the radiation-induced maxillary sinusitis. These patients received sequential sinus X ray examinations, sinuscopy and biopsy of the maxillary sinus mucosa before, immediately after and 2 months after radiotherapy. These three examinations were used to evaluate the maxillary sinuses. The results show that Sino Vent insertion could not prevent the development of maxillary sinusitis induced by radiotherapy. On the contrary, it had adverse effect. Basides, we also found that the mucosa of an irradiated maxillary sinus became edematous and polypoid, then it gradually returned to normal after radiotherapy. |
