中文摘要 | 傳統治療關節炎之中藥林林總總,但許多藥物 (尤其是湯劑) 一般而言 ,使用于 臨床治療急性或較嚴重之關節炎如痛風或類風濕關節炎等效果不佳。而急性關節炎或類風濕 關節炎其發炎或疼痛之有關作用與前列腺素或活化之 T 細胞有關, 本研究基于此擬進行體 外實驗,先行瞭解各種常用治療關節炎方劑其是否有抑制中性白血球釋放前列腺素或淋巴球 釋放 IL2 之能力。 本研究中,擬選用古方中常用以治療關節炎方劑包括蕙苡仁湯、麻杏薏 甘湯、葛根湯、桂枝芍藥知母湯、上中下通用痛用丸、芍藥甘草湯、疏☆ g 立安散等, 經 過傳統抽取法 (將由盛昌公司提供 ),以乾燥粉劑或溶液分別與從正常人分離出之中性白血 球 (l X 1O �� cell/ml, 以 A 23187 作為刺激物 ) 及淋巴球 (5 X lO �� cells/well, 以 PMA 作為刺激物 ) 分別作用 30 分鐘及 2 天後, 取出上清液, 再以 Enzyme immunoassay kit (Cayman 公司 ) 測試中性白血球釋放前列腺素之濃度及 ELISA kit (Genzyme, USA't 測試淋巴球釋放 IL2 之濃度,最後再相互比較加入中藥與同組不加中藥 其是否有明顯差異。 結果顯示,雷公藤具體外實驗濃度相當低 (0.6-2 μ g/ml) 皆有明顯 抑制 T 淋巴球釋放 PGE2,然其它中藥則需相當高的濃度 (100-500 μ g/ml 方有較明顯抑 制 IL2 或 PGE2 作用。 從上述結果瞭解雷公藤因其有明顯地抑制 PGE2 (類似 NSAID 及 IL2 (類似 cyclosporin),故可作為治療抗風濕免疫的藥物,且療效應較快,而其它傳統之 抗風濕藥物則需相當大之濃度才可達到抑制作用,故臨床上其抗風濕免疫作用療效較差,且 作用應較慢。未來擬在進一步探討不同之抗風濕中藥其療效與作用機轉。 |
英文摘要 | There are many varieties of traditional anti-rheumatic herbs but usually, those herbs are not effectively for acute and severe arthritis or immune diseases. In this study, we have used the in vitro experiment to understand whether the popular anti-rheumatic or immunosuppressive herbs had a significant effect on either the prostaglandin E2 (PGE2) release from the neutrophils or the interleukin 2 (IL2) release from lymphocytes. Except for T2 (Tripterpium wilfordii Fobtained from Taichou pharmaceutical company, Mainland), 7 other herbs including Yih Yii Ren Tang, Ma Shing Yih Gang Tang, Ger Gen Tang, Guey JyShaur Yaw Jy Mu Tang, Shang Jong Shieh Tong Yong Tonp Fens Wan, ShaurYaw Can Tsao Tanp, and Shu Jin Lin an Saan (obtained from Shen Chung herb Company) were either ectracted from ethanol or water. The herb was adjusted into different concentration from 0.1 to 500ug/ml and incubated with mononuclear cells from 3 healthy persons. The stimulant is PHA. To test the PGE2 release from neutrophils, the herb incubated with neutrophil and the stimulant was A23187. The results show T2 at lower concentration) 0.6-2.Oug/ml) can signifcantly inhibit both IL2 and PGE2. In contrast, other herbs did not present a striking effect until the herb reached in a high concentration (100-500ug/ml). In conclusion, T2 and be taken as an antiinflammatory agent like NSAID and also as an immuno-suppressive drug like cyclosporin. However, other antirheumatic herbs must take a large dosage to give a therapeutic effect. We hope in the future, many anti-rheumatic herbs can be studied scientifically. |
