中文摘要 | 在1992年之前,台灣並沒有使用B.O.P.人工合成骨於頸椎前位融合術之經驗。我們從1992年10月起至1996年7月止,共施行了83例前途之手術,其中男性53例,女性30例,平均年齡50.5歲,平均追蹤期是28個月。從我們的經驗中發現,B.O.P.要達到良好頸椎融合所需的時間遠比腸骨(iliac bone)來得長,此外,由於B.O.P.的硬度大,所以殖入位置的上下椎體如果因為骨贅形成(osteophytosis)太厲害而需要積極磨去骨皮質的話(aggressive docortication) ,往往會造成日後椎體本身的蹋陷。另外,由於B.O.P.融合耗時太長,在骨折一脫位的病人不建議使用。並且一且在術後追蹤發現B.O.P.移位脫出,則必須趕緊再次手術移除,以免造成食道破裂的併發症。 |
英文摘要 | Anterior cervical spine operation with interbody grafting has been well known for over forty years in the treatment of diseased intervertebral discs, osteophytic compression or cervical spine injury. Several kinds of graft material have been used in cervical arthrodesis, and most surgeons prefer a graft taken from the iliac crest of the patient. However, the problems related to the iliac graft such as pain, hematoma, infection or fracture of the donor site have led a number of surgeons to start using allografts, heterografts or synthetic grafts. The Biocompatible Osteoconductive Polymer (B.O.P.),one of the synthetic grafts, was reported to be an excellent substitiute for interbody grafts in parts other than the cervical spine and was introduced into Taiwan in 1992. The authors reviewed 83 patients undergoing anterior cervical discectomies and interbody fusions with B.O.P.grafts over a 3-year follow-up, and have attempted to determine whether the overall benefit surpasses the potential risk from using such grafts. The present study as well as reports from other authors shows that advantages of the B.O.P.graft are its firm strength, biocompatibility and availability. But it appears that the incidence of complications such as collapes of vertebral bodies due to aggressive decortication or graft dislodgments due to delayed union is higher in patients applied with B.O.P.grafts than those with iliac grafts. As regards the choice of a graft material in anterior cervical fusion, the authors are firmly convinced that the B.O.P. graft is not the first choice even though it has some advantages. |
