中文摘要 | 遠距會診可以幫助偏遠地區的醫師確定診斷,減少轉診的需要,並幫助病患在當 地就醫,提昇偏遠地區基層醫療照顧水準。本研究報告臺大醫院與臺北縣金山鄉衛生所使用 互動式遠距會診系統, 進行家庭醫學科照會之實施成果。 研究期間總共照會 320 人次,96% 以上的患者認為利用本系統能聽的清楚及看得清楚會診的醫師,然而也有 9.0% 患者認為會 診醫師不能了解他們的病情。 轉診醫師 98.7% 認為利用遠距會診溝通的效果可以接受, 94.9% 認為發生誤解的頻率為完全沒有或只有一兩次。 會診醫師 99.7% 接受本系統的溝通 效果,89.3% 認為發生誤解的頻率為完全沒有或只有一兩次。80.4% 轉診醫師完全認同會診 醫師的建議,10.5% 部分認同,沒有人完全不認同。 本研究的結論認為使用互動式遠距會診系統進行遠距照會為實際可行的方式,患者、轉診醫 師及會診醫師的滿意都很高。然而如何維持良好的醫病關係,確認診斷治療的正確性是仍待 研究的課題。 |
英文摘要 | This study is to evaluate an interactive tele-consultation system for consulation between National Taiwan University and Chinshan Health Care Center. There were 320 teleconsultations during the study period, mopre than 96% of the patients considered that they were able to see and hear the consulted doctors clearly through this system. There were 9.0% of the patients considered their consulted doctors could not understand their illness condition. 98.7% of the referring physicians and 94.9 of consulatants involved in tele-consulation reported that they accepted the communication quality. The main purpose of the tele-consultations (94.6%) was to seek the second opinions from consultants. Thus, 80.4% of the diagnosis suggestions made by the consultatants were totally accepted by the referring physicians, 10.5% were partially accepted, and none was totally rejected. From the study, it may conclude that the interactive tele-consultations system would be practical in terms of the communictaion quality and the acceptability of the patients, referring physicians and consulatnts. However, the questions of how to miantain good doctor-patient relationship and to improve the accuracy of diagnosis using the system are the subjects for future study. |
