中文摘要 | 我們的前期實驗已敘述燒傷血漿可同時增加骨骰肌及肝對胺基酸的吸 收,此存於血漿中的因子既非荷爾蒙,亦非cytokineS,極有可能來自燒傷本身的 特有物,本實驗即在測試燒傷結肺是否為因子的來源;我們以結麻下存液 (subescharfluid,為試驗物。測其對骨骼肌胺基酸吸收的影響。 (1)燒傷面積在百分之三十以上的二至三度燒傷患者,於燒傷後七十二小時內,抽 取結麻下存液(subescharfluid),並將之離心儲存。(2)小老鼠在麻醉下取下兩側比 目魚肌,置於培養皿申(內含結伽下存腋及放射性胺基酸,-AminoisobutyriCacid) 培養兩小時,取出肌肉,測其放射值。13'計算及結果統計分析。 我們的發現有三個:(1)燒傷結痂下存液可降低骨股肌對胺基酸的吸收。(2)隨燒傷 面積的增加,所抽取的燒傷結痂下存液,對骨骼肌胺基酸的吸收的抑制也增加。 (3)燒傷後抽取結痂下存液的時間愈後,其對骨骼肌胺基酸的吸收的抑制也大。 我們結果顯示結痂下存液中確實存在某不明因素,可明顯減少骨骼肌對胺基酸的 吸收;同時此因素與前期實驗中燒傷血漿中的因子不同,宜繼續深入分析此因素。 |
英文摘要 | Inhibition of amino acid transport in skeletal muscle after thermal injuryhas been demonstrated previously. The present study investigated theeffects, in vitro, of subeschar fluid from burned victims on amino acid transport in incubated soleus muscles. Subeschar fluid was collected from justunderneath the full thickness of the burn area. Since burn injury mayinduce increased capillary permeability, the subeschar fluid was believedto transude from the capillaries and to contain high level burn skin toxin".Thirteen patients with severe thermal injury (35 to 90 total body surfacearea) entered this study. Subeschar fluid was collected in several procedures, beginning as early as 18 hours after injury and continuing for 4 to 8hours. Amino acid transport was studied with 3H-α-aminoisobutyric acid(α-AIB). Subeschar fluid was added in vitro to incubated soleus musclesfrom healthy animals. Amino acid transport was measured by determiningintracellular uptake of α-AIB during a 2h incubation. α-AIB uptake wasreduced significantly when subeschar fluid was added to incubated muscle.This suppressive effect increased following either burn area increase or thetime interval of subeschar fluid collection after injury. A factor or factor(s)exists in subeschar fluid which depressed muscle amino acid transport,postburn. |
