| 英文摘要 | Retrospective evaluation of the efficacy and the safety margin ofendocapsular cataract extraction and ultra-violet (U-V) blocking intraocularlens(IOL) in patients with high myopic cataract. Seventeen consecutivepatients (twenty two eyes) with high myopic cataract (axial length=26.0 orlonger), eighteen eyes had an axial length longer than 29.0mm. All patientswere treated by modified endocapsular cataract extraction and U-V blockingIOL implantation. Follow up parameters included best corrected visual acuity,keratometry, slit lamp biomicroscopic photography, color fundusphotography, and B-scan ultrasonography. These parameters wereestablished during the postoperative follow up period (from 3 months to 53months). All patients showed improved postoperative visual acuity andwere able to stop using thick glasses. Myopic macular degeneration seemedto be the most important prognostic factor. No Nd-YAG capsulotomy wasperformed, and the involution of Elschnig's pearls was noted. No retinaldetachment was found during the follow-up period. As regards corneoscleralopening in comparison with conventional ECCE, the wound is smaller, thenumber of stitches fewer, the astigmatism less, and the healing better. Theadvantages of the modified "envelope" technique, in comparison withphacoemulsification for excessive myopic cataract are less cost, betterbenefits and safety margin due to its double "safety valve" mechanism. Itmay also ensure the centralization of IOL, resulting in a stabilized capsulezonule-ciliary body complex. |
