| 中文摘要 | 以接觸式G-probe二極體雷射(Diode Laser G-probe),施行經鞏膜睫狀體光 凝固術,自1994年美國食品藥物管理局(FDA)通過後,目前對治療頑強性青光眼, 提供了另一線曙光! 我們使用IRIS醫學儀器公司出品的IRIS G-probe作為二極體雷射之探頭,治療六位 已對藥物、雷射或手術等治療,均告失敗之頑強性青光眼病人,其診斷包含:三 例新生血管性青光眼,一例外傷引起之青光眼,一例慢性隅角開放性青光眼,以 及一例因FuchS'其色性虹膜睫狀體炎引起之青光眼。 手術時,二極體雷射光波長810μm,使用2000~1500 milliwatts,每點雷射光照射 時間為2秒,由於G-Probe特殊的弧面設計,操作時以G-probe的短端接觸輪部處結 膜,此時恰可使G-probe的發射中心在輪部後1.2mm處,我們在輪部上方270度的 範圍中,平均地作20點。觀察六例病患術前和術後三個月的眼壓和用藥數,發現 術後都有統計學上明顯的降低,(P<0.05);而在觀察期間,術後的併發症例如:輕微 灼熱感、虹彩炎等,都可在短期內緩解。此初步研究顯示,以接觸式G-probe二 極體雷射經鞏膜睫狀體光凝固術,對頑強性青光眼治療,具有相當療效,但長期 的效果及併發症,仍待更進一步的追蹤評估。 |
| 英文摘要 | The contact transscleral cyclophotocoagulationdiode laser with G-porbe has been developed as anew method to reduce IOP in refractory glaucoma.We used the IRIS Medical Oculight Diode LaserSystem and the IRIS G-probe to treat 6 patients withrefractory glaucoma uncontrollable by medical, laseror previous surgical therapies. For each laser application, the shorter edge ofG-probe contacted the conjunctiva at limbus. Thecenter of G-probe was 1.2mm posterior to limbus,directly over the ciliary process. 20 applications wereplaced at superior 270 degrees arc. We set the powerof 2,000 milliwatts with the duration of 2 seconds, and kept this procedure just below an audible "snap"or "pop" sound. The mean pre-operative IOP andthe antiglaumatic medications were significantlyreduced (P < 0.05) after 3 months follow-up. The advantages of contact diode transscleralcyclophotocoagulation with G-probe including:(1) An effective and safe technique for IOP control in refractory glaucoma.(2) The minimal and tolerable side effects, such as mild heating and pigmentation of tissue.(3) No serious complication as bulbar atrophy in our short-term study. |
