中文摘要 | 本研究收集民國77年7月至82年6月五年間,因頭部外傷至臺北市21家及花蓮縣4家公私立醫院就診的13∼18歲青少年病歷為研究資料,共收集到臺北市2,001名,花蓮縣449名。在城鄉頭部外傷的發生,男性均較女性高,男女性比例臺北市為2.5,花蓮縣為1.9;年齡均以16∼18歲組(高中階段)較13∼15歲組(國中階段)高;發生原因均以車禍為最主要原因,臺北市81.26%,花蓮縣 87.91%;車禍中又以機車發生率最高,臺北市80.66%,花蓮縣81.87%,而其次在臺北市為行人被撞(11.31%),花蓮縣為腳踏車所引起(8.81%)。本研究發現;青少年因機車交通事故引起頭部外傷之病例,城鄉分別有80.27%及79.34%為無照駕駛;而青少年因飲酒引起意外事故方面,花蓮縣較台北市高4倍(分別為22%及(5.3%);而受傷月份城鄉均以八、九月份,即暑假期間為最高;如將嚴重度與結果之間加以分析,發現重度傷害有較高比例造成死亡或植物人。青少年為國家中堅,因頭部外傷所造成潛在生命、經濟及生產力的損失甚為重大。而臺灣地區又以車禍引起最為嚴重。當務之急應盡快進行無照駕駛、酒醉駕車的取締,並積極宣導安全帽的使用,即可大幅降低殘障與死亡率,未來則應有更積極的、多層面的防治政策來保護我們的青少年。 |
英文摘要 | A study of head injury among adolescents was undertaken from July 1,1988 to.Tune 30, 1993. Clinical records, including in-patient charts, were collected from 21 major hospitals in Taipei City (urban) and 4 major hospitals in Hualien County (rural). A total of 2,001 cases in Taipei City and 449 cases in Hualien County were identified. In this study the incidence, of head injury was much higher in males than in females in both urban and rural area. The male to female ratio was 2.5 in Taipei City an d 1.9 in Hualien County- The incidence rate Was higher in the 16-18 year-old group (senior high school students) than in the 13-15 year-old group (junior high school students). The results of this study showed that traffic accident was the main cause of head injury. In Taipei City 81.26% of head injury cases were due to traffic accidents and in Hualien County 87.91%. In Taipei City most of the traffic accidents involved motorcycle use (80.66%), followed by pedestri-ans (11.31%), and in Hualien County most of the traffic accidents involved motorcycle use (81.87%), followed by bicycle use (8.8l%). This study found that most motorcycle-related head injury cases (80.27% in Taipei City and 79.34% in Hualien County) involved unlicensed riders. The proportion of alcohol-related head injury cases in Hualien County (22%) was four times greater than in Taipei City (5.3%). The num-ber of cases peaked during the months of August and September (during summer vacation). Glasgow Coma Scale (GCS) and Glasgow Outcome Scale (COS) analysis revealed that rates of mortality and vegetative state increased with increasin g severity of injury. |
