中文摘要 | 國人一向有醫食同源的觀念,本研究目的在於了解國內住院患者使用各類特殊營養食品的情形,因而以全省醫院住院病患為對象,據醫院評定等級抽樣調查,以問卷方式共計訪視2,649位患者,其中男性1,469人、女性1,180人,全部納入統計。以各層醫院所得之粗使用率加以加權校正,得出台灣地區特殊營養品使用率,平日未入院時為11.2,而入院後則為16.1,患者入院次數、所屬科別、性別、教育程度均影響使用率的高低,多變項對數迴歸結果指出,住院次數愈多者、五十歲以上者、教育程度為高中以上者、飲食處方為灌食者,是本研究住院患者使用特殊營養食品的主要決定因素。但若加入"平時是否使用",則住院次數與教育程度的影響消失。針對患者所使用的產品予以分析,以蛋白質類補充食品使用人次最多(62.7),其次為完全營養的聚合配方食品(23.8)。產品每日至少食用一次者佔58.6,且已使用一個月以上者達40.3。患者使用產品的「媒介」,來自專業人員者,醫師14.6、營養師17.4、護理人員10.3,均低於患者親友的告知29.3,且食用動機主要是自我認知(36.3)或親友交待(16.0)對病情有所助益。購買場所又以院外超市、一般商店居首(47.4),直銷其次(18.2)。此次調查明顯看出特殊營養食品在患者之間使用廣泛,且未為醫療專業人員所指導。 |
英文摘要 | The purpose of this study was to investigate the use and characteristics of special dietary food consumed by hospitalized patients in the Taiwan Area. Atotal of 2,649 subjects were systematically selected from three grades of hospitals which were stratified by accreditation. Patients were interviewed by trained dietitians or nurses to collect their demographic data and status of special dietaryfood uses before and during the current admission. The weighted prevalences derived from the crude rates of hospitals with different grades were 11.2 and16.1 before and during admission, respectively. Univariately, the number ofadmissions, dapartment of hospitalization, sex, and education level of subjectswere correlated with uasge. The results of the multiple logistic regression indicated of those patients who tend to use such products, admission rate was higher,age was over fifties, education level was senior high school or above, and use oftube-feeding was greater. However, the significance of the number of admissions and education level disappeared after adjusting for preadmission use of theproducts. The protein supplements were used most frequently and constituted 62.7 of all products used. Polymeric formulas were the second-most frequently used products at 23.8. The study showed that 58.6 of the products were usedby subjects on a regular daily basis, and 40.3 had been used longer than one month. Patients reported that the person or group who suggested the use ofthe products was families and friends 29.3 of the time rather than medical professionals (physicians 14.6, dietitians 17.4, nurse 10.3). As to the motivations for usage, 36.3 of the subjects themselves, and 16.0 patients' families believed that the products were beneficial to their recovery from disease. Themajority of products were purchased in supermarkets and grocery (47.4), while 18.2 were purchased through personal sales. This study indicated that special dietary foods were used often by hospitalized patients and their use was not necessarily influenced by the instruction of medical professionals. |
