中文摘要 | 足背神經血管島狀皮辦(Dorsal is pedis neurovascular is- 1 and flap)適用於後腳(Heel)、腳踝(Ankle)及前脛部位(Pretibial region)之皮膚缺損而骨骼肌腱曝露者。因皮瓣薄而且具有固定並易於辨認之血管神經系統。榮民總醫院自民國72年1月至民國75年1月,3年中共有10例因下肢遠端皮層缺損(Skin defect)而接受足背神經血管島狀皮瓣重建之病患。年齡分佈由17至67歲,平均42.5歲。男性7例、女性3例。皮庸缺損由3x 4平方公分至10x6平方公分大小不等。原因以外傷居多。9例於術前足背動脈之搏動極易觸摸。所有病例之後脛動脈(Posterior tibial artery)搏動均易偵測。手術中所有病例之第一蹠骨動脈(First dorsal metatarsal artery)均可找到,因而沒有延遲性手術之病例。手術結果有一例遠端三分之一皮辨壞死,有一例手術後皮瓣於脛骨前部有瘻管形成,有兩例供皮區之植皮後傷口癒合不良,有三例之兩點區別感距離(Two point discrimination)超過20毫米。長期術後追踪外觀及功能,結果皆良好。 |
英文摘要 | Due to its thinness easily identifiable vessels and nerve supply, dorsalis pedis neurovascular island flap is an attractive donor site for defects over heel, malleolus and pretibial region with bony exposure. From January 1983 to January 1986, 10 cases of dorsalis pedis neurovascular island flaps were performed in Veterans General Hospital to provide soft tissue coverage over the distal lower extremities. Age distribution was from 17 to 67 years, with average age of 42.5 years. The ratio between males and females was 7:3. The sizes of skin defects were from 3x4cm to 10x6cm. The dorsalis pedis artery, which has clear pulsation, was easily palpable in 9 of the 10 cases. In a single case the pulsation was weaker but the course still could be easily confirmed by the Doppler flowmeter. Every patient's posterior tibial artery could be palpated easily also. No arteriography was done in this series. The first dorsalis metatarsal artery could be found in every case. None of the cases required delayed procedure. One case showed partial necrosis of the flap's distal area, and another case had fistula formation over the pretibial region, but it healed normally after the removal of the internal fixator. The healing of the donor site was delayed in two cases. Two-point discrimination was more than 20mm in three cases during the two to nine months' follow-up periods. These follow-up examinations showed the durability of the flaps to be good, and sensory recovery was judged satisfactory in this series. |
