中文摘要 | 本研究探討不同肘關節角度對尺神經之運動神經傳導速度,及對小指外展肌上所記錄到之複合肌肉動作電位(CMAP)所產生的影響。共計對19名正常年輕人(24-34歲,男9名,女10名)兩側上肢的38條尺神經,在肘關節完全伸張(FE),屈曲45度(F45),屈曲90度(F90),及完全屈曲(FF)四種姿勢下作電刺激。結果顯示:(1)CMAP之潛期(latency),振幅(amplitude of negative phase)及面積(area)不因肘關節角度改變而改變。(2)跨肘段尺運動神經傳導速度(AcV: across elbow MNCV),前臂段尺運動神經傳導速度(FV: forearm MNCV)與跨肘段距離長度(D)都隨著肘關節屈曲角度增加而呈有意義增加。(3)在FE, F45, F90, FF四個不同肘關節角度下,(AcV/FV×100%分別為83.40%, 81.20%, 102.16%, 108.53%。(AcV/FV)值隨肘屈曲角度增加而變大,其影響之主要因子為各角度下量得的跨肘段距離長度(D)隨著肘屈曲角度增加而增加而前臂段距離則不因肘角度增加而改變。 本研究除針對以上結果分析探討,同時建立不同肘關節角度下FV, AcV,左右兩側AcV差絕對值之平均值及標準差(meam±SD),及AcV/FV值。以此為基礎,期望以後在檢查不正常個案時能更進一步分析在何種肘關節角度下檢測敏感度最高,以尋求往後作肘部尺神經病變電學診斷時肘關節角度姿勢之標準化。 |
英文摘要 | The purpose of this study is to investigate the effect of elbow position on segmental motor nerve conduction velocity (MNCV) of ulnar nerve and compound motor action potential (CMAP) pick up on abductor digiti minimi. The study was performed on 38 ulnar nerves of 19 healthy persons under 4 different joint angles (FE: full extension, F45: flexion 45°, F90: flexion 90°, FF: full flexion). The results show: (1)The latency, amplitude and area of CMAP are all not affected by elbow position. (2)The ulnar MNCV of across elbow segment (AcV) and forearm segment (FV) and the measured length of across elbow segment (D)increased with the flexion angle of elbow joint significantly. (3)The mean values of (AcV/FV)×100% under the elbow positions FE, F45, F90 and FF are 83.40%, 91.20%, 102.16% and 108.53% respectively. The increasing value of AcV/FV is related to the increasing elbow flexion angle. The most important affecting factor is the length of the across elbow segment (D), which is also related to the increasing elbow flexion angle, but not the forearm segment. Except investigating and analyzing the data, we establish the value of AcV/FV and the "Mean and Standard Deviation" of AcV, FV and absolute value of difference between left side and right side. According to the study, we expect to standardize the elbow position in detecting the ulnar neuropathy around the elbow. |
