| 中文摘要 | 磁振造影之使用已有相當多歷史,但是真正應用在骨骼肌肉系統,最近有五至七年才逐漸趨於普遍。其中最主要的關鍵因素在於磁振造影能夠形成非常好的軟組織的比較效應(soft tissue contrast)以及非常好的解析度(resolution),同時磁振造影可以依任何角度對人體作掃描,不會因為病人姿勢的美制而造成某種角度無法成像,所以磁振造影對於骨骼肌肉系統的各項疾病,無論是軟組織、韌帶、軟骨或骨內的疾病都有相當明確的分析。 其中應用最普遍廣泛的是膝關節的成像,包括韌帶的受傷、軟骨的受傷、半月軟體斷裂以及骨內骨小樑斷裂,或是骨髓水腫等等變化,這些是以前X光檢查非常難加以診斷的。其次,為肩關節及腕關節的成像,對肩關節及腕關節的一些非常細微及三度空間的構造能夠清楚的表現出來,診斷出微小的肌腱炎或是肌腱斷裂,同時對於腕關節的腕骨之間的關係,更能夠明顯的表現。 對於脊椎的磁振造影亦有相當大的突破,病人可以不必接受侵入性的脊椎影像,不但對於脊髓本身的疾病,對於脊髓外之脊髓膜以及骨骼方面的疾病,也有相當大的突破。最後,對於骨髓成像,尤其是血液性疾病或者是惡性腫瘤的骨髓侵襲能藉著磁振造影而早期發現其變化。所以,磁振造影在骨骼肌肉系統的應用,可以說是非常的廣泛而且正確。 |
| 英文摘要 | Magnetic resonance image (MRI) are common applied in the musculoskeletal disease in recent five to seven years. It is most contributed by the better soft tissue contrast and resolution. On the other hand, the MRI can image the patient according to any anatomical plane without changing the patient’s posture. Thus, the MRI has quite good and accurate diagnosis and analysis of musculoskeletal disease, such as articular cartilage, ligaments, soft tissue or intraosseous bony lesion. The knee image is the most common application of MRI, including ligamentous injury, cartilaginous injury, musical injury and intraosseous bone marrow edema. These diseases are quite difficult to e diagnosed by using concentional radiographic examination. Secondly, MRI is also important in shoulder and wrist examination. MR can demonstrate the very fine and sophisticated anatomical structures in three-demotion demonstration. It also can diagnose the small tendinitis v.s. tear. MRI also have great contribution in showing the relationship of carpal bone of wrist. The MR of spine has quite great advantage for the patient, the patient can have noninvasive examination instead of such an invasive myelography examination. MR can have very clean image analysis of spine, not only the spinal cord, but also the dural and extradual bony lesion. MR also have very good in formation about the bone marrow imaging, esp. for the hematological disease and bone marrow infilliation by malignant rumors. MRI can have very early detention of the bone marrow anormaly. Thus, the application of MRI in musculoskeletal system is very broad and accurate. |
