中文摘要 | 假性血管瘤與血腫塊都是因血管受傷出血造成的脈動性腫塊,確定假性血管瘤的診斷對於擬定治療計劃非常重要,血管攝影雖然可以診斷假性血管瘤,但它是一種侵襲性、具游離輻射且需注射顯影劑的檢查;在本研究中有27例脈動性病灶包含假性血管瘤17例和血腫塊10例經彩色杜普勒超音波診斷確定,彩色超音波的灰階影像波顯示假性血管瘤的大小、位置及壁血栓的厚薄,彩色影像及杜普勒頻譜表現出假性血管瘤的血流動力學的兩種特徵--連接病灶與動脈的通道內有「來來往往」(to-and-fro)的血流及病灶內漩渦狀血流所造成的yin-yang sign或color washing machine sign,此種現象在每一個假性血管瘤的病例皆有。彩色杜普勒超音波是一種非侵襲性、快速且正確的診斷假性血管瘤的方法,將會取代血管攝影成為標準的診斷工具。 |
英文摘要 | Both pseudoaneurysm and hematoma are pulsatile mass that results from leakage of blood from the disrupted vascular wall into the soft tissues. The diagnosis of the pseudoaneurysm is very important in therapeutic plan. Angiography can make diagnosis but it is invasive study with radiation exposure and contrast injection. Color Doppler ultrasound was used to identify pulsatile masses of extremities in 27 cases, including 17 pseudoaneurysms and 10 hematomas. The gray-scale image shows the size, location, and thickness of nural thrombus. The color Doppler image and Doppler spectrum depict two hemodynamic characteriatics; the first is "to-and-fro" flow in the channel between lesion and artery, the second is swirling flow within the cavity of lesion called "yin-yang sign" or "color washing machine sign". Color Doppler ultrasound is a non-invasive, rapid, and accurate method for the diagnosis of pseudoaneurysm. In the future, it might replace the conventional angiography as a standard tool for the diagnosis of pseudoaneurysm. |
