| 中文摘要 | 晶體上皮分化規則排列纖維,並維持其內離子平衡以保持清澈透明。本 文藉電顯觀察,期比較不同程度白內障上皮細胞之超微細構造。六例罹不同程度 老年性白內障,其囊膜於手術施行前囊撕裂術時獲得。另有壹完整晶體,由19 歲之捐贈眼球取得。結果顯示於輕度及未熟白內障,上皮細胞形態規則並呈立方 形。細胞問明顯摺疊,結合緊密,胞質含多量粒腺體及粗內質網等。惟這些胞器 不如年輕晶體明顯。胞核則位於中央,核染質呈分散狀。成熟白內障之上皮細胞 主要特點在於細胞形狀呈扁平,胞器無特殊變化,僅胞質intermediatefilament;增 多。過熟白內障之上皮呈極度退化,細胞形態變薄及扭曲。細胞之間嚴重剝離, 所有胞器幾為空泡取代,胞核呈溶解狀態。前囊膜下白斑則為胞外基質及散在其 中之退化紡錘狀細胞組成。 本研究顯示未熟晶體上皮細胞之微細構造,與年輕晶體無明顯差異;即使在成熟 白內障之上皮,僅形狀呈扁平變化,細微構造變化亦不大。其扁平變化,歸諸於 膨大晶體之壓迫所致。而過熟白內障之上皮,則呈嚴重萎縮。因上皮功能主在調 整晶體內之離子及水分平衡,當該層細胞逐漸退化,皮質纖維中將漸次產生空 泡、水裂、水球等混濁。而當上皮完全失償時,晶體將發生過熟而完全混濁。本 研究推論上皮之驟然退化導致白內障過熟期之形成。然其形成機轉,則有待未來 進一步研探。(慈濟醫學 1994;6:95-101) |
| 英文摘要 | In order to elucidate the ultrastructural variations of lens epithelial cells associated with theseverity of cataract, six human lens epithelium specimens (three from immature cataracts, one froma mature cataract, and two from hypermature cataracts) and one whole lens from the eye of a19-year-old cadaver were examined and compared under the transmission electron microscope.Results show that, when compared with a normal young lens, the ultrastructure of the epitheliumin mild cataractous lens appeared to be relatively normal intracellularly. In a mature cataract,the cell became flattened and increased interdigitation of the opposed cytoplasmic membrane andintracytoplasmic intermediate filaments were noted. On the other hand, the lens epithelial cell ina hypermature cataract manifested marked attenuation and atrophy in cell shape and remarkabledegenerative changes in the intracellular organelles. Furthermore, fibrous pseudometaplasia withabundant extracellular matrix was sometimes seen .under the lens capsule, which coincides withthe fibrous plaque preoperatively observed under the slit lamp. With the propagation of lensopalescence, the lens epithelium not only loses its cell mass but also deteriorates at the ultrastructural level, which aggravated mostly at the advanced stage of cataract.
