中文摘要 | 心臟外科手術中,人工心肺循環的運轉有其必要性,但同時對於血球細胞又會造成不可避免的損傷,例如影響紅血球變形能力、血液灌注、及凝血機轉等,因此容易引起較多的手術併發症。 在本研究,在14個開心並接受人工心肺循環的病人中,我們以紅血球變形能力,作為紅血球受傷的指標,從而探討verapamil在體外實驗中,可否改善人工心肺循環對紅血球變形能力的傷害。 以該病人未經人工心肺循環的紅血球變形能力為參考。在對照組,經過人工心肺循環60分鐘後和循環結束時(平均循環時間:150.9±12.1分鐘),其紅血球變形能力平均分別下降了41%和65%,而在verapamil這組,變形能力平均分別只下降了10%和35%。相較兩組,不論在人工心肺循環60分鐘後或結束時,verapamil這組顯示紅血球變形能力的受損減輕了不少,P<0.05。由本研究中,我們可確定verapamil在體外實驗中,可保護紅血球對抗人工心肺循環所造成的傷害。 |
英文摘要 | In 14 patients undergoing open-heart surgery, red cell deformability was assessed by Reid's microfiltration method. The deformability was expressed as red cells filtration rate (RFR) in μl/s. Mean RFR values showed a generally falling tendency after cardiopulmonary bypass (CPB). Compared with the preoperative values, significant decrease in RFR values were seen 60 minutes after CPB (by 41%) and at the end of CPB (by 65%),respectively. After two hours' incubation with verapamil ( 1μg/ml ), the RFR values decreased much less than those of the control group ( p<0.05 ). The percentages of RFR reduction in the verapamil group were 10% 60 minutes after CPB and 35% at the end of CPB as against 45% and 65% respectively in the control group. This study confirmed the deleterious effect of CPB on red cells and showed that its damaging effect can be significantly reduced by use of verapamil in vitro. |
