| 中文摘要 | 本實驗是以台大物理治療系三、四年級的58位學生及剛畢業的年輕老師 為研究對象,探討直接接受短波熱療與間接接受短波熱療,治療部位的表皮膚溫 度、血管直徑、口徑面積、血流速及血流量各有何不同的反應。以受試者的右膝 為接受感應線圈式短波熱療的實驗部位。實驗之前、後各以溫度測試儀 (Thermometer,TS120ThewoSentry)測量左右兩腿膝蓋骨正中央,膝上15公分,膝下 10公分共3點的表皮膚溫度;另以複合杜卜勒超音波儀測出左右兩側股動脈、膕動 脈的血管直徑、口徑面積、單位時間的血流速及每分鐘的血流量之大小。每位受 試者各接受2次測試,一次是右膝熱療時左膝貼近右膝,另一次是右膝熱療時左 膝遠離右膝30公分,以減少右膝短波電磁場對左膝的作用,兩次測試至少相隔一 個星期。實驗結果發現:雖然只單右膝熱療,但兩側腿部的表皮膚溫度皆會有意 義上升(p<0.01)。另外,直接熱療側(右腿)的股動脈血管口徑、面積、血流速、血 流量都大量增加。但間接熱療側(左腿)的反應大不相同:(1)右膝熱療時左膝貼近 者,血管之變化不大。(2)右膝熱療時左膝遠離右膝30公分者,股動脈血流速、血 流量都銳減。(中華物療誌1993;18(1):64-71) |
| 英文摘要 | The aim of this study was to investigatethe differential effect of direct and indirectheating by applying shortwave diathermy onsingle knee. All of 58 experimental subjects arephysical therapy students or junior teachingassistants. They do not have cardiovasculardiseases and any musculoskeletal or neuromuscular disorders of their lower extremities. Theaverage age of them is 22.4±0.9 y/o ; theaverage height of them is 167.6±3.7cm ; theaverage weight is 66.4±2.4 kg. All of theirright knees received shortwave diathermy withinductional cable twice with one week apart.Initially the left knee was kept in close to righknee during heating, then one week later theleft knee was kept 30 cm distant from rightknee when receiving shortwave diathermy foreliminating the direct electromagnetic effect ofshortwave. The two experiments were arrangedat an interval of at least one week to avoidresidual heat effect. Before and after receivingshortwave diathermy, skin temperature of bilateral lower extremities at central patella, abovepatella 15cm and below patella 10 cm weremeasured by a thermometer. The vessel diameter, its area of cross section, the time averagevelocity and total volume of blood flow offemoral and popliteal arteries were also re corded by Doppler ultrasound(Diasonics DRF400). The data were analyzed by paired-t test.This study demonstrated the following results :1. Direct and indirect shortwave heating in creased skin temperature significantly in both lower extremities. The direct heating cannot raise skin temperature more than indirect heating and it made no difference whether both knees were separated or not.2. Increase of femoral arterial diameter, its area of cross section , average velocity, and total volume of blood flow per minute was sig nificant in direct heating side (right knee). Some subjects showed negative response and the decrease of blood flow was significant, too.3. There were two different responses with in direct heating. Increase of vessel diameter, area of cross section, average velocity, and total volume of blood flow was not signifi cant in subjects with both knees closed. Decrease of average velocity and total vol ume of blood flow per minute was signifi cant in subjects with both knees separated.4. The effect of direct heating by shortwave diathermy may not be substituted by indirect heating. (JPTA ROC 1993; 18(1): 64-71) |
