中文摘要 | 本報告利用流動分析儀技術共回顧性研究了108例臘塊包埋膀胱癌腫瘤組織DNA之含量。男女比例為87比21。平均追蹤時間為50個月研究發現臨床行為,包括五年存活率,復發率及疾病進展皆與組織病理細胞之分化及腫瘤期別有密切相關性。分析結果腫瘤DNA含量異常者佔了56%。依細胞分化等級分,第一級分化良好者,其DNA異常之機率僅12%,第二級分化中等者為49%,第三級分化差者則機率高達81%。表淺性腫瘤,Ta(O)及Ti(A)期別其DNA含量異常之機率為36%及54%,而肌肉層有侵犯之膀胱癌者,T2及T3-4之DNA含量異常機率則皆高達86%。同一腫瘤組織內出現多發性非套數染色體母細胞株之情況並不與腫瘤分化或期別相關。染色體套數正常與否和病人五年存活率,復發率及疾病進展有密切關連。中等分化(第二級)膀胱癌可根據流動細胞分析之結果再區分為二組不同預後之病人群。因此我們認為除了傳統之病理組織檢查外,DNA流動細胞分析可再提供膀胱癌病人另一有用之預後指數。 |
英文摘要 | Cell material from 108 cases of archival paraffin-embedded tumor specimens of newly-diagnosed transitional cell carcinomas of the urinary bladder was studied retrospectively, using rapid flow cytofluorometric DNA analysis. The male-to-female ratio was 87/21. The mean follow-up time was 50 months. Clinical behaviors, including survival, recurrence, and progression, were related to the histopathological grading and tumor staging. The tumors were classified as diploid ( DI=0.9-1.1 ) or aneuploid. Total aneuploid occurrence was 56%. The aneuploid frequencies in varying grade tumors was 12% of Grade 1, 49% of Grade 2, and 81% of Grade 3. Stage Ta (O) and Tl (A) tumors differed from muscle-invasive tumors ( T2, T3-4 ) in that they had a lower frequency of aneuploid occurrence (36%, 54% vs 80%, 86%). Multiple aneuploid stem cell lines had no influence on tumor grade and stage. The DNA ploidy was closely related with five-year survival rates, the probability of tumor recurrence, and progression. Bladder tumors of intermediate grade (grade 2) may be subdivided into two groups with different outcomes on the basis of flow cytometric characteristics. It is concluded that DNA flow cytometry can serve as an additional prognostic parameter for bladder cancer patients in addition to conventional histopathology. |
