中文摘要 | 本研究計畫係針對有關內毒素注射與腹內膿瘍造成之敗血症在致死機轉上可能存在之差異做動物實驗,期望以此了解是否臨床上之敗血症(如腹內膿瘍)含有其他非源自於內毒素之致死因素。實驗選用C3H/HeN及C3H/HeJ老鼠做為實驗動物。選用這兩種動物的理由是二者是很接近的品種,而前者對內毒素具有如其它動物一般的反應,會因為注射內毒素而產生組織破壞及死亡;但後者,則因基因突變具有抗內毒素之特性,不會因注射內毒素而發生組織破壞或死亡。比較這兩種動物於腹內膿瘍造成之敗血症時的死亡率及組織器官受到破壞的形式所得到的知識,有助於了解此種敗血症與內毒素實驗模型之差異。實驗方法是以盲腸結紮及穿刺(CLP)造成腹內膿瘍及敗血症(經血液培養證實)。實驗結果發現,雖然C3H/HeJ老鼠具有抗細菌內毒素的特性,然而於CLP引致之敗血症時,發生與C3H/HeN老鼠相同的死亡率;不過,兩種動物因敗血症所產生的組織破壞,在組織病理學上,有明顯不同的變化。在C3H/HeN老鼠,產生組織形態學上有明顯變化的器官有肺臟,腎臟及小腸;在C3H/HeJ老鼠則只見腎臟及小腸的病理變化,肺臟的病理變化則缺如。與既知注射內毒素或Tumor Necrosis Factor(TNF)所產生的組織變化相比較。C3H/HeN老鼠於敗血症時所產生的組織破壞形態十分接近,但C3H/HeJ老鼠的則不同。這些實驗結果使我們推論腹內膿瘍造成之敗血症可能含有其他異於內毒素之致死因子,這個推論可以說明為什麼具有抗內毒素特性的C3H/HeJ老鼠並沒有於腹內膿瘍造成之敗血症時發生較低的死亡率,同時也可以解釋它不同的也織破壞形式。 |
英文摘要 | To investigate the difference between endotoxin-injection models and clinical sepses such as intra-abdominal abscess, this study was undertaken to examine whether the latter contains toxic factors other than endotoxins. C3H/HeN and C3H/HeJ mice, used for the experiment, are two very similar strains. However, unlike the C3 H/He N strain, C3 H/He J mice, a mutant variant of the former, are insensitive to endotoxins because of a genetic defect; they are able to survive endotoxemia at levels lethal to the C3H/HeN strain. A comparison of mortality rates and tissue injury patterns induced by sepsis secondary to intra-abdominal abscess in these two animals may provide knowledge useful for distinguishing the difference between endotoxin-injection models and sepsis caused by such etiology. Cecal ligation and puncture (CLP) methods were used in the experiment to produce sepsis (proved by blood cultures). The results were that, although C3H/HeJ mice were resistant to endotoxin toxicities, mortality rates caused by CLP-induced sepsis did not differ from those of C3H/HeN mice (p>0.05). Histopathological studies revealed that in C3H/HeN mice, injuries were prominent in the lungs, kidneys and small bowel but, in C3H/HeJ mice, only kidney and samll bowel injuries were obvious. Pulmonary injuries in the latter were minimal. On comparison with known tissue-injury patterns elicited by endotoxin or tumor necrosis factor (TNF) injection, those of C3H/HeN mice produced by CLP were very similar; those of C3H/HeJ mice were not. These results led to the postulate the presence of toxic factors other than endotoxins during sepsis from intra-abdominal abscess. This would help explain the unmitigated mortality rates of endotoxin-resistant mice in CLP-induced sepsis, and provide a reason for the different injury pattern of tissues. |
