中文摘要 | 白內障手術後引起之正散光會隨著時間漸漸回歸,且有許多因素會影響 到它的散光變化,每位醫師習慣的手術切割方式可能不盡相同,了解到個人手術 後散光回歸情形是相當垂要的。 收集145例行襄外白內障摘除術後植入後房人工水晶體之病例,所有病人之手術 均由同一位醫師施行,且縫合之材料及手術技巧均相同,以自動電腦驗光機來檢 查術後散光之變化情形,散光之變化因考慮到軸度變化而採用向量方法測之。 術後追踩超過七個月以上之病人其平均散光回歸為3.08±1.31D,術後最初之散光 在約一星期左右,平均為正散光2.62±1.54D,至最終約18個月以上時,平均為逆 散光-0.37±1.20D。術後8至10星期前為散光變化最快速之期間。術後初期散光愈 大,整個散光回歸也就愈大。術後早期散光若維持在3.0D左右應是很理想的。 術前之散光若在1.0D以上時會影響到術後之散光變化,此時須有術前之散光為正 或逆散光而來修正切割與縫合之方法。 |
英文摘要 | Surgically induced with tlie rule astigmatism after cataract surgery regress with time and many influencing factors can affect its results. It is important to determine tlie cylinder regression profile for your specific incision size and architecture. 145 cases were collected after extracapsular cataract extraction and posterior chamber lens implantation. All the operations were done by a surgeon with same operative technique and suture material. Automatic refraction was used to estimate the cylinder clianges from early postoperative days. Cylinder changes as regard to axis were considered and analyzed by vector method. Mean individual cylinder regression was 3.08 ± 1.31D in cases followed for more than 7 months. Postoperative mean cylinder was initial, about 1 week, 2.62± 1.54D (with the rule) and final, more than 18 months, -0.73 ± 1.20D (against the rule). The fast cylinder decay was noticed before 8 to 10 weeks postoperatively. Larger intial cylinder resulted in more cylinder regression. To reach initial witli the rule astigmatism around 3.0D is satisfactory. Preoperative astigmatism more than 1.0D affected the results of postoperative cylinder changes. Alterations of incision and suture methods are needed in those cases with regard to preoperative with or against the rule astigmatism. |
