中文摘要 | 以台灣獼猴為動物實驗對象,共8隻成猴施行兩側半月板切除術 (meniscectomy),右側不置任何植入物,而左側移植腹部皮膚當作關節盤取代物,術後第4,8,12及24週各犧牲2隻動物,取出兩側顳顎關節標本,先以放射線檢查再固定於福馬林,脫鈣之前以慢速骨鋸 (Isomet) 連續切成厚度3mm薄片,再以14%EDTA脫鈣,經石臘包埋,切取4μm切片,用hematoxylin和eosm染色,觀察比較各時期左右顳顎關節之組織反應。 整體而言,在左側顳顎關節內之植入物往外側移位且有退化之現象,兩側關節腔皆因纖維粘連而變小,但左側比傾向顯著較少。在4週和8週之標本中可見髁頭及關節隆凸後方喪失纖維軟骨,左側較右側不顯著。相反的,在12週和24週時,髁頭和關節隆凸後方有纖維軟骨增厚,特別是左側關節。在所有之動物均可觀察到關節窩及關節隆凸前方有新生骨形成。 由此研究顯示,半月板切除後會產生纖維粘連,然而,以皮膚移植置入其中似可延遲甚或避免關節粘連。 |
英文摘要 | The main purpose of this study is to investigate the changes in the temporomandibular joint (TMJ ) after meniscectomy, with/ without interpositional skin grafting. Eight adult Taiwanese monkeys (Macaca cyclopis) were ernployed in this study. Afterbilateral meniscectomies. skin graft obtained from the abdomen was implanted into the left joint to replace the removed articular disc and the graft was secured to the surrounding tissues with sutures. No implantation was done on the right side. Two animals were sacrificed at each of the postoperative periods of 4,8,12 and 24 weeks. The whole-TMJ specimens on both sides were obtained, radiographed and fixed in 10% formalin. Before decalcification, the specimens were serially sectioned into 3mm thick slices with a low-speed bone saw (Isomet) . Deca1cification was performed in 14% EDTA. After paraffin embedding, sectioning into 4μm and staining with hematoxylin and eosin, the specimens were carefully examined and compared with each other to discern the histopathological reactions. Generally, the grafts in the left TMJs were found to be laterally displaced and degenerated in all animals studied. The joint cavities of both sides diminished in size due to fibrous adhesion in all animals, but this tendency was significantly less on the left side. In the 4 and 8-week specimens, loss of fibrocartilage was noted on the condylar head and posterior slope of the eminence but these changes was less marked on the left side. In contrast, the 12 and 24-week specimens had fibrocartilage thickening on the condylar head and posterior slope of the eminence, especially of the left side. In all animals, new bone formation was obvious in the glenoid fossa and anterior aspect of the eminence. The results in this study demonstrated that after meniscectomy fibrous adhesion would ensue readily. However, interpositional skin graft seemed to delay or even prevent the development of joint adhesion. |
