中文摘要 | 本研究目的在於瞭解膝關節手術後不同時期的病人,患側膝部肌力與姿 勢變化之關係,並與健側比較。共有22位膝關節手術後,未接受肌力訓練的病患 參與本實驗。有部分病患在手術後不同時期,接受一次以上追蹤檢查,因此共有 38筆資料;受試者分為膝關節鏡後與開膝開節術後兩組。每位受試者以Cybex II等 速肌力測驗儀,在5個不同之膝關節角度與髖關節角度下,變換25個姿勢以測試 不同姿勢下之最大膝伸肌與膝屈肌之等長收縮肌力。結果顯示關節位置改變對健 側伸肌與屈肌之肌力表現方式,其正常者相同;即肌肉被擺在長度較長之位置 時,肌力表現佳,且相對於其他姿勢之肌力表現呈現統計上有意義之差異 (p<0.05)。其中膝伸肌肌力在髖關節越彎曲的位置相對膝關節越伸直下作圖,可 看出隨著肌肉長度縮短,呈現明顯之向下階梯型模式而患側伸肌肌力表現方式則 有不同之改變,其表現受到距離手術後之時間長短的影響。所有受試者在手術後 一個月內,姿勢改變幾乎不影響肌力之表現;所有姿勢下所測得之肌力力矩值, 彼此間都沒有顯著差異,肌力相對於姿勢變化之圖形是齊頭型模式。距離手術後 一個月以上之患側伸肌肌力,則漸越近於健側伸肌肌力受姿勢影響之方式。臨床 上對手術後之病患進行肌力測試與訓練時,在不同時期是否應採取不同之姿勢, 有待更進一步之研究。(中華物療誌1992;17(2):149-157) |
英文摘要 | The purposes of this study are to find therelationship of position change and muscletorque of knee muscles in post-operative patients and to compare the muscle torque of theoperative knee with it of the unaffected knee,Twenty-two post-operative knee patients participated in this study. They were tested withCybex II dynamometer for maximum isometricstrength of knee extensors and flexors. Thetests were done at five hip positions - hip flexion 90o, 70o, 50o, supine and prone, paired withfive knee positions - knee flexion 90o, 60o, 45o,30o and 0o. The subjects were divided into postarthrotomy group and post-arthroscopy group.Each group has three subgroups for the variouspost-operative periods (group 1: 1-4 weeks,group 2:5-26 weeks, group3: 27-104 weeks).The results showed that the unaffected knee ex tensor at more lengthening position could produce larger torque than it did at shorteningposition. The difference was statistically significant (P<0.05). A step-down tendency was alsofound in knee extensor torque at various kneepositions from knee flexion 90o to 0o. Thisphenomenon was also shown in group ofhealthy subjects, but was altered in the affectedside. In the first month post-surgery, changingof the positions does not influence the performance of muscle torque at all. There was nosignificant difference existed within the valuesof isometric muscle torque at all testing positions.
