英文摘要 | Paired transthoracic and transesophageal echocardiography (TIE, TEE) were per-formed on 26 patients referred for the assessment of suspected infective endocarditis. Each paired study was reviewed by 2 observers blinded to clinical data. Studies were graded as positive or negative for vegetations. Using clinical and pathological data, 16 of 26 (61.5 %) patients were positive for infective endocarditis. TEE found 11 patients with a single vegetation: 7 had a vegetation at the mitral valve, 3 had a vegetation at the aortic valve and one had a vegetation at the pul-monary valve; and 3 patients with multiple vegetations: two patients had vegetations at both anterior and posterior mitral leaflets and one patient had vegetations at the mitral, aortic and pulmonary valves. Two patients with mitral valve prolapse who had redundant anterior mitral leaflet were over-diagnosed as having a vegetation by TIE but not by TEE. The sensitivity of TTE and TEE was 56.3% (9/16) and 87.5% (14/16) respectively (p < 0.001) in detecting the infective endocarditis. Specifity for the detec-tion of infective endocarditis by TTE and TEE was 80% versus 100%, and the accura-cy was 65.4% versus 92.3%, respectively. In conclusion, TEE is more sensitive and specific for the detection of intracardiac vegetation than TIE. It is particularly useful whenever the TTE fails to confirm the clinically suspected case of infective endocarditis. (J Emerg Crit Care Med. 1992;3(1):25-36) |
