中文摘要 | 31例患單純心室中膈缺損病重在心導管檢查證實24小時以內,做雙面彩色杜蔔勒超音波心圖檢查,測定左至右心室分流面積。以分析錄影帶記錄收縮尖期、舒張期、舒張末期及最大左至右分流面積,與心導管檢查測得左至右分流大小,及收縮尖期、舒張期及舒張末期左右心室間壓力差及收縮期壓力差總和比較,結果發現左右心室分流與最大分流面積成正相關(p=0.05),與經體表面積及心跳校正的最大分流面積成正相關(p<0.001),與經體表面積收縮尖期分流面積成正相關(p<0.001),而左與右心室間收縮尖期壓力差與經體表面積校正的最大分流面積成反比(p<0.05),與經體表面積及心跳校正的收縮尖期分流面積成反比(p<0.05)。雙面彩色超音波心圖不僅能正確地診斷心室中膈缺損的位置及大小,而且能提供血流體力學變化的資料。 |
英文摘要 | To evaluate the efficacy of two-dimensional Doppler echocardiography in assessing the severity of isolated ventricular septal defect, 31 children were studied within 24 hours of cardiac catheterization. The shunt flow area at peak systole (PSFA), diastole (DFA) and end-diastole (EDFA) and maximal shunt flow area (MSFA) were measured with frame-by-frame technique and corrected with body surface area and heart rate. The Qp/Qs ratio was calculated by the Fick's principle. Simultaneous pressures at peak systole (PSP), diastole (DP) and end-diastole (EDP) were recorded in the left (LV) and right ventricle (RV). The Qp/Qs ratio was correlated well with MFA (p=0.05), MFA/BSA (p<0.001), PSFA/BSA (p<0.001), MFA/HR (p<0.05), MFA/BSA, HR (p<0.001), PSFA/ BSA and HR (p<0.001). The PSP gradient between LV and RV was inversely correlated with MFAIBSA and PSFA/BSA (p<0.05) and HR (p<0.05). Two-dimensional Doppler echocardiography can provide a useful information of hemodynamic changes in children with isolated ventricular septal defect. |
