中文摘要 | 本研究目的在於評估頭頸部腫瘤患者於放射治療中之營養狀況,並探討血清前白蛋白(serum prealbumin)是否適宜作為短期的營養指標;同時觀察於積極營養支持時,血清前白蛋白之變化,並和白蛋白(albumin),運鐵蛋白(transferrin)等與營養相關的生化數值作比較。評估對象為30位頭頸部腫瘤患者,平均年齡46歲。於其大範圍放射治療期間,平均26次約五週中,每隔一週測其前白蛋白、白蛋白、運鐵蛋白、血清含鐵量。每週測體重並估計該週平均熱量攝取百分比。熱量不足者,予以建議口服補充品或鼻胃管灌食。隨著放射線劑量累積與副作用增強,熱量攝取量漸少,大部分生化數值有逐週下降趨勢,但均在正常範圍內。以ANOVA 分析五週中各參數的變化情形,發現前白蛋白的降低有顯著差異,顯示前白蛋白較白蛋白、運鐵蛋白、體重為更有效的短期營養指標。同時前白蛋白也和熱量攝取有關,當熱量攝取增加,前白蛋白降低的較少;熱量攝取減少,前白蛋白降低的較多。 |
英文摘要 | The purpose of the study was to evaluate the nutritional status of patients with head and neck cancer during the course of radiotherapy, and also to investigate the feasibility of serum prealbumin as an effective short-term nutritional indicator. The changes in the serum prealbumin level under nutritional support were determined and a comparative study was done with albumin and transferrin. Thirty cases of head and neck cancer were selected randomly, excluding those with liver or renal diseases, disorders of the thyroid gland, trauma, surgery, and thrombosis of blood vessels. The mean age was 46.1 years. During the first five weeks of radiotherapy, determination of serum prealbumin, albumin, transferrin and serum iron was done every other week. Measurement of body weight and evaluation of the average caloric intake percentage were done every week. Oral supplement or tube feeding was given to those with inadequate intakes. With the accumulation of the radiation dose, the side effects would become more pronounced and oral intake would decrease gradually. Most of the values of the parameters decreased week by week, but all remained within normal range. The sequential changes of the studied nutritional parameters during radiotherapy was analyzed with ANOVA, and the changes in prealbumin were found to be statistically significant. Prealbumin was found to be a more sensitive and effective nutritional indicator than albumin, transferrin or body weight. Prealbumin was positively correlated with caloric intake. |
