中文摘要 | 自從低能量雷射發展成功以後,醫用雷射應用的範間由切割、燒灼、凝固、蒸發等擴充到更廣的領域,其中之一即為組織的融合(welding)。許多方面的嘗試,包括輸尿管、腸道、輸卵管、血管、皮膚……等組織的吻合,已經獲得初步的成功。由於在顯微鏡下吻合細小血管是整形外科醫師必須具備的技術,常應用於斷肢重接、皮瓣移植等重建手術。因此,我們以動物實驗評估此種方法與傳統縫合方法的差別。根據對50隻大花鼠腹主動脈(直徑約1.0至1.5毫米)以兩種方法吻合的結果,顯示以低能量二氧化碳雷射輔助血管吻合,至少在三方面較傳統方法為優:一、縮短手術時間。二、減低出血量。三、較優的組織學變心。尤其是後者,咸信對細小血管的長期流通率(long term patency)有決定性的影響。因此,我們認為此種方法將在顯微血管吻合術上佔一席重要的地位,甚至取代傳統的方法。目前,我們已初步將之應用於臨床,有令人鼓舞的結果。 |
英文摘要 | Fifty Long-Even mice (weighing 100-200 gm) were divided into two groups, experimental and control, using the abdominal aortae (1.0-1.5mm) as Laser- Assisted Microvascular Anastomosis and conventional suture anastomosis models. The statistical differences of performance and the quality of the anastomosis were compared between these two groups. Advantges of the LAMA over the conventional suture method included shortened operating time, decreased bleeding, and better histological results. The goal of this study was to investigate the clinical feasibility of this new technique. Special interest was focused on whether this method could become a better alternative to the conventional suture anastomosis. It is possible that this new expertise will gain acceptance for use in microsurgery, but no conclusion can yet be made. However, initial trial on a few patients is being encouraged at present. |
