中文摘要 | 本文之研究對象為55名臨床主訴為胸痛而接受了冠狀動脈攝影術之病人,55名病人亦接受了運動心電圖術以及運動放射核種血管造影術。55名病人中經證實41名患有冠狀動脈疾病而14名其冠狀動脈正常。正常組左心室射血分率在靜止及最大運動時分別為66±7.0%及74±8%(p<0.005),而疾病組織左心室射血分率分別為60±14%及55±17%(不具統計學的差獲)。陽性之運動放射核種造影檢查定義為運動後左心室射血分率沒有增加5%或產生運動引發之心壁運動異常。結果顯示,運動放射核種血管造影之靈敏度與專一性分別為85%與86%,而運動心類圖術之靈敏度與專一性分別為55%與100%。此外,未達適當的運動量會降低運動放射核種血管造影術之靈敏度。我們認為運動放射核種血管造影術對於診斷冠狀動脈疾病具有相當不錯之靈敏度與專一性,同時運動量是否適當則影響檢查之靈敏度至鉅。 |
英文摘要 | Fifty five patients who underwent coronary angiography for evaluation of chest pain received exercise radionuclide ingiography and exercise electrocardiography. There were 14 patients with normal coronary angiograms and 41 patients with conronary artery disease. The left ventricular ejection fraction in rest and exercise radionuclide angiography were 66+7% vs 74+8% in control group (p<0.005) and 60+14% vs 55+17% in patients with coronary artery disease. A positive exercise radionuclide angiography was defined as the failure of increase of 5% of ejection fraction or development of new or exaggerated wall motion abnormalities. The overall sensitivity and specificity in exercise radionuclide angiography vs exercise electrocardiography were 85% and 86% vs 56% and 100%. In addition, patients with inadequate exercise level has lower sensitivity than patients with adequate exercise level. We conclude that: 1) Exercise radionuclide angiography is sensitive and specific for detection of coronary artery disease, 2) Inadequate exercise level is one of the most important reasons which influence the sensitivity of the test. |
