中文摘要 | 生物敷料運用在灼傷傷口上,已有多年經驗,並證實它的臨床價值。但很少有生物敷抖覆蓋在耳朵灼傷的文獻報告。我們將"羊膜"作輻射試菌消毒,以利長期保存及運用。 首先,我們在紐西蘭種大白兔二度灼傷的耳朵,以三種不同方法治療:輻射滅茵後羊膜覆蓋,全抹silver sulfadiazine cream,凡士林紗布色紮。觀察察肉眼及組織切片檢查。結果羊膜組優於其吃兩組。在肉眼檢查,羊膜組的傷口較乾燥,消腫較快,癒合較好。 所以我們以輻射滅菌羊膜覆蓋二十八例二度灼傷耳朵的病患上。羊膜薄、透明、柔軟、沾黏性好。運用於病患灼傷耳朵,也是傷口較乾燥、消腫快、疼痛減輕、癒合較好,沒有羊膜下膿瘍生成。所有病患在半年至兩年半追踪檢查期間,汲有化膿性軟骨炎的併發症,外觀恢復正常。 經輻射誠菌的羊膜,價錢便宜、使用方使,可長期保存。它能減輕疼痛,促進傷口癒合,是二度灼傷良好的生物敷料。尤其在處理構造凹凸不平耳朵灼傷時,更具有明顯的療效。 |
英文摘要 | Several years of clinical experience have demonstrated that biological dressings are very beneficial for the treatment of burns. However, there are very few reports in the literature on the use of this treatment for burned ears. Experiments were conducted on second-degree burns to the ears of New Zealand rabbits with three different types of dressings: radiosterilized amnion, silver sulfadiazine cream, and vaseline gauze. Results were compared by both gross and microscopic examination. Under gross examination, wounds were drier, swelling subsided faster, and healing progressed well. Under microscopic examination inflammatory reaction was reduced and reepithelization took place faster. Amnion was clearly the superior dressing from the comparison studies. Therefore, radiosterilized amnion was used to dress 28 cases of second-degree burns to human ears. In these cases as well radiosterilized amnion was found to be thin, transparent, pliable; it adhered well. Wounds were drier, swe1ling subsided faster, pain was reduced, and healing progressed well and no subamnion abscesses formed. Each case was followed for from six to thirty months; there was neither suppurative chondritis nor deformity of the burned ear. Radiosterilized amnion is cheap, easily prepared, and easily preserved for long periods. It reduces pain and improves healing. It is an ideal biological dressing, particularly effective in the treatment of such intricate structures as the ear. |
