| 中文摘要 | 32例先天性心臟病病童(含16例心室中隔缺損,5例心房中隔缺损,5例複雜發紺型,6例法洛氏四合症羣)及5例風濕心臟病童在心導管檢查的24小時以內同時做周邊對比劑M型及杜蔔勒超音波心圖研究,以分析杜蔔勒超音波心圖,診斷由右向左分流對比劑之效果。對照組的所有5例風濕性心臟病病童在左側心臟告無M型或杜蔔勒超音波之對比劑廻音。在5例心房中隔缺損病童中,所有病例有3例經對比劑杜蔔勒超音波心圖檢查顯示於左心房可發現對比劑。於16例心室中隔缺損病童中,有4例M型及杜蔔勒超音波心圖皆在左心室內有右向左分流之對比劑。而6例法洛氏四合症羣及5例複雜發紺型先天型心臟病童在左側心臟以此二法皆可發現由右至左分流引起的對比劑廻音。本研究報告顯示以對比劑超音波心圖檢查在主要為由左向右分流之先天性心臟病患者,仍可能有少量之右向左分流存在。並杜蔔勒超音波心圖比傳統之M型超音波心圖更易於偵測此少量由右向左分流存在。 |
| 英文摘要 | Thirty-two patients were studied who had various congenital heart diseases including isolated ventricular septal defect (VSD), 16 patients; isolated atrial septal defect (ASD), 5 patients; tetralogy of Fallot (TOF), 6 patients; complex congenital heart disease and oxymetry documented right o left atrial shunts, 5 patients; and rheumatic heart disease (RHD), 5 patients. All received peripheral contrast M-mode and pulsed Doppler echocardiographic studies (PDE) within 24 hours of cardiac catheterization for evaluation of right-to-left shunts and comparison of the sensitivity between contrast M-mode and PDE. Of the five patients with ASD, two had PDE evidence of contrast in left atrium. This small amount of contrast was not detected by the M-mode study. Of the 16 patients with VSD, 4 patients had the contrast in the left ventricle detected by both M-mode and PDE studies. There was no false positive contrast in the left atrium or left ventricle among the five control patients with RIID by either M-mode or FDE study. The six patients with TOF and five patients with complex cyanotic congenital heart diseases had right-to-left shunts detected by both M-mode and PDE studies. PDE-contrast echocardiographic study serves as a more sensitive method for detection of a small amount of right-to-left contrast, especially in cases of ASD. Thus it can be a supplementary, non-invasive aid for evaluaion of intracardiac shunring, useful in addition to the traditional M-mode contrast echocardiography in patients with congenital heart diseases. |
