中文摘要 | 依陰部神經大腦皮質誘發電位,可測出其神經傳導時間與誘發電位幅度,其波形極為穩定的M型,本研究以164位神經學檢查正常的人,先刺激其陰莖背部神經,在顱頂Cz處可測得其誘發電位,其神經傳導時間onset是33.21±2.69 ms,P1是38.84±2.62 ms,N1是47.52±2.82 ms,P2是57.89±3.17 ms,N2是70.58±4.15 ms,P3是83.97±8.09 ms,其誘發電位幅度onset-P1是0.31±0.22 uv,P1 - N1是0.82±0.58 uv,N1-P2是1.16±0.79 uv,P2-N2是1.40±0.88 uv,N2-P3是1.48±0.88 uv,此幅度愈往後愈大,本誘發電位之神經傳導時間不因年齡及身高之大小而有所改變,其數值極為穩定,但是幅度隨年齡之增大而變大,其原因未明。 任何原因引起陰部神經或支配膀胱或性功能之副交感神經有病變時,都有可能改變這些神經的傳導時間與電位幅度,因此對於神經性膀胱及神經性性功能失調的診斷極為重要。 |
英文摘要 | The pudendal nerve supplies motor innervation to the external anal and urethral sphincters, pelvic muscles and the bulbocavernosus muscle. It also receives sensory information from the penis, urethra and anal sphincters, and the pelvic floor muscles. The urinary bladder is innervated by the sacral cord (S2-4). The dorsal nerve of the penis (a bninch of the pudendal nerve) is stimulated and averaged the evoked potentials over lumbosacral cord and sensory cortex to measure the conduction time (latency)and amplitude of the evoked potentials. The central and peripheral integrity of these nerves is important for normal sexual, bladder and defecation functions. This study evaluated 164 neurologically normal males, aged 55.37±10.79 years old. Their heights were 167.61±6.55 cm. The cortical evoked potential of the pudendal nerve demonstrated a typical M pattern. The latencies of onset, Pl, Nl, P2, N2, and P3 are 33.21±2.69 ms, 38.84±2.62 ms, 47.52±2.82 ms, 57.89±3.17 ms, 70.58±4.15 ms, and 83.97±8.09 ms respectively. The amplitudes showed gradual increase from onset-Pl to N2-P3. There were no significant changes of latency in individuals of different age and body height. The lumbosacral-evoked potentials and evoked potential from the lower urinary tract were difficult to obtain in this series. Any dysfunction of these neural pathways may cause abnormal changes of latency or amplitude of the cortical evoked potential. It can provide a valuable tool to differentiate between neurogenic and non-neurogenic lesions in sexual and vesical dysfunctions. |
