| 中文摘要 | 我們比較了亞硝酸鹽試驗,白血球酯酶試驗,及尿液格蘭氏染色檢查等做為尿液培養結果的篩選效能之評估。在受檢的270支尿液檢體中,75支(27.8%)的培養結果為有意義之菌尿,其中57支檢體為培養樂白血球酯酶或亞硝酸鹽試驗均為陽性(敏感性為76%)。在195支培養結果為陰性的檢體中,119支為培養及白血球脂酶或亞硝酸鹽試驗均為陰性(專一性為61%),白血球酯酶或亞硝酸鹽試驗對陰性連液培養之預期值為86.9%。而尿液格蘭氏染色檢查結果其敏感性、專一性和對陰性尿液培養結果之預期值分別為100%,79%,及100%。我們建議可以尿液格蘭氏染色檢查為尿液培養篩檢試驗。 |
| 英文摘要 | Sediment microscopy and physicochemical reactions, both employed as screening techniques for urine cultures, were evaluated. Of 270 urine specimens specimens screened, 75 were judged to have clinical significance: 57 showed both culture and leukocyte esterase/nitrite positive (sensitivity=76%); 195 were judged not to have clinical significance: 119 showed both culture and leukocyte esterase/nitrite negative (specificity=61%). The predictive value of negative result of leukocyte esterase/nitrite test were 86.9%. Compared with Gram stain, of 75 culture positive specimens, 75 showed both culture and Gram stain positive (sensitivity=100%); of 195 culture negative specimens, 154 showed both culture and Gram stain negative (specificity=79%). The predictive value of negative result of Gram stain was 100%. The Gram stain is more efficient in predicting an negative culture result (X2=11.819, p<0.001) than leukocyte esterase test. |
