| 中文摘要 | 近年來自於車禍和意外傷害日益增加及經皮膚腎臟手術的流行,使得腎臟外傷的織會大大的提高,尤其是多處外傷之病人往往比較危急,因以如何能迅速且正確地診斷是相當重要。目前我們常使用的檢查方法有靜脈腎盂攝影、超音波、血管攝影和電腦斷層檢查等,其中電腦斷層檢查是一個相當精確且迅速的工具,對於腎臟受傷的程度它能提供相當詳細的訊息,且對於腹腔內及後腹腔器官做一個篩檢似地檢查提供我們治療的方向。 近兩年來在本院的連續100個腎臟外傷病人中,臨床上懷疑有嚴重的腎臟外傷患者共有40人,其中30人成功地接受保守治療,有7人接受手術治療,3位病人自於有腎內血管病變而成功地接受動脈血管栓塞術。 在40位病人當中,有3位手術後及12位未接受手術之患者在2-6個月內回來追踪檢查效果均十分良好,3位病人由於腎臟受到相當嚴重的傷害而接受腎摘除,1位病人由於腹部嚴重壓榨傷而死於全身器官功能衰竭。 根據國外的報導及我們的經驗電腦斷層檢查有相當好的診斷價值,因此我們建議若病人懷疑有重度的腎臟外傷,應馬上接受電腦斷層之檢查以節省時間爭取治療時效。 |
| 英文摘要 | An increase in traffic accidents and percutaneous renal surgery have resulted in more kidney injuries, An immediate diagnostic evaluation of renal injury to document its severity and degree is necessary. This should inc1ude excretory urography, nephrotomography, sonography, arteriography and computerized tomography (C-T). Among these, C-T is the most accurate, rapid and relatively noninvasive modality. It provides most precise anatomical details of renal injuries as well as valuable information regarding the condition of other retroperitoneal and intra-abdominal structures which can often result in the early identification of associated, unsuspected injuries. Among the 100 consecutive renal injuries observed in the recent 2 year, 40 were evaluated with C-T when major kidney damage was suspected. Of these patients 30 were managed successfully with conservative treatment, 7 underwent surgery, and 3 patients developed pseudoaneurysm or arteriovenous malformation after percutaneous nephrolithotomy and received superselective arterial embolization. 12 nonsurgical and 3 surgical patients were followed up by C-T 2-6 months later. This investigation revealed complete recovery of the kidney contour and complete resorption of hematomas. The use of C-T is advocated as a primary diagnostic modality in patients suspected of sustaining major renal injury and/or injury of other organs. This approach has facilitated greatly both diagnosis and treatment of kidney injunies. |
