英文摘要 | To assess the, effects of a brief period (3 minutes) of ventricular distention after high potassium cardioplegic arrest, 12 normal and 8 chronically volume hypertrophied canine hearts were studied. The hypertrophied models were developed by chronic volume overload with a descending aortic to left atrial shunt (6 mm Gore-Tex) for at least 28 days. Each of the studied hearts was protected during ischemia by hypothermia (l5℃)and high potassium (20 mEq) cardioplegic arrest. Each heart was then resuscitated and at 10- minute intervals, isovolumic left ventricular (LV) developed pressure and diastolic pressure were measured with an intra-ventricular balloon. In the normal heart after 100 mmHg distention, there was a marked diminution of LV function, 81±9% of control, as well as a decrease in LV compliance, 75±l0% of the control. However this decrease in both left ventricular function and compliance resolved with continued reperfusion and resuscitation in the beating, non-working left ventricle to 95±8% for LV function, and 90±9% for compliance, after 30 minutes. However, in the hypertrophied volume-overloaded ventricle with even minor LV distention of 50 mmHg, there was a significant decrease in both LV function and compliance. There was no recovery at 30 minutes (LV function 75±9%, compliance 69±l0%, p=0.005). These data demonstrate that the normal ventricle can withstand severe distention during the rest period. However, moderate ventricular distention (50 mmHg), even for a brief period of time in the chronic volume hypertrophied ventricle, produces both marked depression of ventricular function and decreased compliance which were not reversible during the reperfusion period. |
