中文摘要 | 榮民總醫院神經外科自73年9月起採用西德Aesculap出產之卡氏合成金屬骨(Caspar osteosynthetic plate)對不穩定性頸椎外傷病人實施早期之頸前固定及椎間骨融合手術(interbody fusion)。其優點為可同時達到復位,減壓及穩定三種目標,所以可免除病人長期顱骨牽引的痛苦,而且可早期開始復健治療,使得病人的神經功能障礙之恢復機會增加。 |
英文摘要 | The anterior approach to the cervical spine described by Cloward, and the various fusion technique of Smith-Robinson and Bailey, have been widely used in the management of cervical disc diseases. However, the anterior plate stabilization technique for treatment of unstable cervical spine injuries has not found wide-spread use even though it is optimally advantageous from the standpoints of pathological anatomy, physiology, an operative approach and stabilization. Aesculap cervical spine instruments and Caspar osteosynthetic plates in different lengths are designed particularly for interbody stabilization of one, two or even more cervical spine segments from C2 to T1. Using these instruments can achieve very good surgical exposure of the interbody space, thus facilitating operative procedures. In principle, there are three goals in management of cervical spine injury. The first is to reduce the displaced segment; the second is to decompress the cord or root; and the third to insure stability. By using Caspar osteosynthetic plate fixation and interbody fusion, these three goals can be immediately, simultanesouly achieved at the same time. This is a valuable method for treatment of unstable cervical spine injuries. Since September 1984, seven cases with unstable cervical spine injury have been treated with Caspar osteosynthetic plate fixation and interbody fusion with good effect. The instruments, advantages, indications and surgical results are discussed. |
