| 中文摘要 | 近年來呼吸生理學之進展及儀器製作技術之發達,使最大呼氣流量容積曲線成為極具有潛力之研究肺呼吸機械學的一檢驗方法,而漸有廣泛應用之趨勢。有關健康之成年中國人之最大呼氣流量容積曲線之研究國內已有好多篇文獻報告,但對於少年人及孩童之最大呼氣流量容積曲線之研究,國內尚無人報告過,因此在本研究中我們分析203位國小及國中之孩童之肺功能。其中94位是男生,109位是女生,年齡分佈從10歲至16歲不等。我們利用Hewlett-Packard Pulmonary Calculator System來測試孩童之最大呼氣流量容積曲線,同時並用HP47305 DCOs[92ba] Controller來測試孩童之肺瀰散量。測試結果顯示,此203位孩童,從其最大呼氣流量容積曲線測試中所得之FVC, FEV₁及PE均隨年齡、身高及體重之增加而增加,其呈明顯之正相關,其相關係數從0.595至0.858不等(r=0.595~0.856,P<0.001)而FEF[8e44]-□亦與身高及體重呈明顯之正相關(r=0.650,P<0.001;r=0.595,P<0.001)而與年齡未呈明顯之關係(r=0.127,P>0.05),FEV₁%由於其變異係數(coefficient of variance)甚小,故與身高、體重及年齡均未呈明顯之相關。肺瀰散量亦與年齡、身高及體重呈正相關,相關係數從0.558至0.928不等(r=0.558~0.928,P<0.001)。從203位10~16歲男女孩童之通氣功能及肺瀰散量資料分析,就男孩方面約可分為三階段,分別為10~12歲,13~14歲,15~16歲。三階段之間均有明顯差異。女孩方面亦可分為三階段,分別為10~13歲,14~15歲及16歲以上,各階段之間亦呈明顯差異。男女生各經複相關及多元回歸分析以及年齡及身高為變數,獲得一系列肺功能變數(parameters)之回歸方程式,其式均可如下式表示: 預測值=aA+bH+c (a、b、c均為係數), (A=age, H=height) |
| 英文摘要 | 203 normal school children, 94 boys and 109 girls, ranging in age from 8 to 16 years, were studied to establish the normal values of parameters of maximal expiratory flow-volume curves and the diffusing capacity (D[8ecf]CO). The parameters obtained from the flow-volume curves including forced vital capacity (FVC), one-second forced expiratory volume (FEV₁), FEV₁/FVC ratio, forced expiratory flow rate at 25-75% FVC (FEF□) and the peak flow rate (PF). The parameters increased with age in both boy and girl student groups and the mean values of the parameters of the 94 boy students are higher than the 109 girl student group. The mean value of the diffusing capacity in boy student group was higher also. FVC, FEV₁, FEF□, PF and D[8ecf]CO in both study groups correlated positively with the weight, height and the age. However FEV₁/FVC had no correlation with the anthropometric data. From this study, we may derive the regression equations of the ventilatory function parameters and the diffusing capacity of the 10 to 16 years old normal children, so that the predicted values may be easily obtained. |
