中文摘要 | 本文就施行全人工髖關節47例做術后功能評估及X光變化之追蹤研討。 術后追蹤長達2 1/2年至8年,均用舊型Muller型。病人半數以上為骨性關節炎及缺血性股骨頭壞死。兩例已重行開刀,其中一位為Klebsiella Pneumoniae感染,行Girdlestone術式。另有一例,近日亦準備再接裝。功能上,結果優良者有40例(85.1%)。 X光所見,亦以透明線存在者最為常見,在髖臼側透明線大於2 mm者有21例(44.68%),在股骨側側身5例(10.63%),另外在人工關節與骨泥間有透明線存在者有8例(17.02%)。 若按Gruen17氏之失敗模式則type Ia 2例,type Ib 7例,type II 4例, type III及IV 各1例。異位生骨7例(14.9%),均屬class I、II、III,故無活動上之限制。 由已知之經驗及知識,吾人認為要使全人工關節達到更長之使用年限,應(1)講究骨泥裝填之技術及設備。(2)改換設計或材料更理想之人工關節。(3)重現術后之復健工作。 |
英文摘要 | This is a 2 1/2 to 8 years follow-up study of 47 cases of total hip replacement using conventional curved-stem Muller type prostheses. CMW cement was inserted by digital packing. Osteo-arthritis and avascular necrosis of the femoral head out-numbered other disease entities. This suggested the younger age group of the present series. There was a case of revision of the acetabular component and a case of Girdlestone procedure for the infected total hip due to klebsiella pneumoniae. One more case will have revision of the acetabular cup in the near future. Using modified Harris hip rating system, 40 cases (85.1%) have good to excellent result, 4 cases fair, 3 cases poor. The most common x-ray finding was the presence of radiolucent line between bone and cement. On the acetabular side, there were 21 cases (44.68%) with radiolucent line larger than 2 mm. On the femoral side there were 5 cases (10.6%)with line larger than 2 mm. Also 8 cases (17.02%) have lines between prosthesis and cement. According to Gruen's modes of failure, there were 2 cases in type Ia, 7 cases in type Ib, 4 cases in type II, 1 case in type III and 3 cases in type IV. The high incidence of radiolucent line is the potential threat to the longevity of the prosthesis. A better understanding of the biomechanical and biomaterial knowledge is a basic requirement for cementing technique and prosthetic selection. |
